The purpose of this solicitation amendment is to re-open the solicitation, re-address Quote Preparation Instructions, and to provide a new deadline for submissions. Solicitation Number 36C25025Q0269 is hereby re-opened as most of the submissions received by the Government were incomplete due to missing documentation that was requested in the Quote Preparation Instructions . Â Guidance or feedback will not be provided as to whether your quote was lacking any of the supplied documentation. Please compare your submission with the instructions below and ensure that your response includes all of the Administrative, Technical, Pricing, and Past Performance elements. QUOTE PREPARATION INSTRUCTIONS / INSTRUCTIONS TO OFFERORS To assure timely and equitable evaluation of quotes, offerors must follow the instructions contained herein for quote submissions. Quotes must be complete, self-sufficient, and respond directly to the requirements of this solicitation. FAILURE TO PROVIDE ANY OF THE INFORMATION REQUESTED IN THIS SECTION MAY RESULT IN YOUR QUOTE BEING EXCLUDED FROM FURTHER CONSIDERATION FOR AWARD. All quoters shall submit the following: Administrative Items: Provide a statement concurring or acknowledging the solicitation and any applicable solicitation amendments individually listed by amendment number. It is the offerors responsibility to track applicable amendments and submit additional acknowledgements that may be posted after submission of the contractor s quote. A Table has been provided below as an example. (Failure to provide this certification will result in the offeror being immediately excluded for award). Amendment Number Dated Solicitation N/A Amendment #0001 Amendment #0002 Amendment #0003 Amendment #0004 Cover Page with business name, SAM UEI, Business Address, Point of Contact with Phone and email information. List of all Vendor Points of Contact that will be used to support this requirement. Please provide full name, email addresses and phone numbers along with their roles (Billing, Sales, Transportation, etc.). Any waste hauling permits that are required by the vendor s local jurisdiction (Federal, State, Local). Pricing items: A Completed Price-Cost Schedule which addresses all items for all periods of performance. Technical Items: Provide a proposed Start-up and phase-in schedule to describe how long it will take to have a container on-site. Please also describe what the Government must do to arrange pickups (hauls). Provide detailed contingency plan in the event of equipment failure or transportation problems, which will be utilized to assure contractor compliance with delivery schedules stated in the Performance of Work Statement. Contractors Contingency Plan to include backup production facilities and/or plans for use in the event of failure of contractor s equipment and/or main production facility. This Contingency Plan shall also include transportation vehicle failure. Provide a narrative that clearly and convincingly indicates experience working this type of requirement. Please include the following reports as samples: Recycle Proceed Document TASK 1 SUBTASK 2 Haul report as described in TASK 2 SUBTASK 2 Please list all / any subcontractors if they will be utilized for this requirement AND what they will be performing. If no subcontractors will be used please state No Subcontractors will be used for this requirement Past Performance: Provide at least 3 references to other similarly sized and relevant contracts or requirements that you have performed in the past 3 years. Contract information, including start dates, ultimate completion date, and total contract value. POC, Contracting Officer, COR, or Technical POC name and contact information including phone number and email address. A brief description and narrative on services performed. All quotes shall be sent to Josh Kovar Joshua.Kovar@VA.gov via email. Please note: Any vendors who wish to submit an UPDATED quote shall provide a complete quote package in its entirety. All documents shall be labelled with the words UPDATED 0003 in their file names. Any vendors who do not wish to provide an updated quote package will be evaluated based on their original response, however they need to provide an acknowledgement of amendment 0003 via either email or attachment.
