Dementia Community Engagement Plan

1. Description of Project

The Minnesota Department of Health’s (MDH) Healthy Brain Initiative Team requests proposals to guide the development of a Dementia Community Engagement Plan. This Plan will describe actions to guide state agencies and organizations on how to engage with people living with dementia across the state. This Plan will also demonstrate how people living with dementia can be active participants in ongoing conversations that may influence programming, funding, and policies.

The desired outcome of this contract is to develop the Dementia Community Engagement Plan draft for Minnesota. The contractor should involve people living with dementia to guide the development of this plan. MDH staff, the Healthy Brain Partnership members and key stakeholders will continue to engage and participate in the development of this Plan. The contractor is also expected to present and demonstrate to key stakeholders across Minnesota about the value of engaging with people living with dementia.

This work may be completed virtually. This work should be completed by June 30, 2026.
