Event Description
The purpose of this Request for Proposal (RFP) is to solicit qualified proposals to support the South Dakota Department of Health (SD DOH), Office of Chronic Disease Prevention and Health Promotion (OCDPHP), and the Nutrition & Physical Activity Program in reducing the incidence of overweight, obesity, chronic diseases, Alzheimer’s disease, and other dementias through evidence-based interventions. The successful applicant will present innovative yet proven strategies to influence and improve health behaviors among South Dakotans that impact chronic disease and brain health. The contractor will collaborate closely with SD DOH staff to advance current worksite wellness priorities by coordinating, supporting, and enhancing wellness initiatives statewide. Responsibilities include providing leadership, technical assistance, education, and resources to worksites implementing comprehensive wellness programs focused on chronic disease prevention, physical activity, nutrition, mental well-being, brain health, and overall employee health.

The State will to respond to offeror’s inquiries (if required) via e-mail. In addition, all inquiries and the State’s response will be posted on the state’s e-procurement system. Offerors may not rely on any other statements, either of a written or oral nature, that alter any specification or other term or condition of this RFP. Offerors will be notified in the same manner as indicated above regarding any modifications to this RFP.
