REQUEST FOR QUALIFICATIONS
Sideline Doctor (High School Football Team Physician)
Notice of Solicitation
Notice is hereby given that pursuant to the provisions of N.J.S.A. 19:44A-20, New Jersey Pay to Play, and other
legislative enactments, more specifically Chapter 271 of the laws of the State of New Jersey, the West New
York Board of Education located at 6028 Broadway, West New York, New Jersey 07093 is seeking RFQ's for
professional services to be provided to the Board of Education as listed below for the 2026-2027 School Year.
Introduction and Purpose
The West New York Board of Education is soliciting Request for Proposal from interested persons and/or
medical groups for the provision of High School Football Team Physician to the Board of Education Autism
Program. The successful respondents shall have experience in providing a broad range of sports-injury
related treatment services to New Jersey public schools and education communities.
The successful respondent will provide the West New York Board of Education with a program to act as the
district's designated High School Football Team Physician and provide Sideline preparedness services,
including the identification of and planning for medical services to promote the safety of the athlete, to limit
injury, and to provide medical care at the site of practice or competition, and related services as required. The
successful respondent will be duly certified in the state of New Jersey with active licenses, including NJ. Med.
License, IMPACT Certified (concussion management) licensed to practice in the State of New Jersey, will
assure board policy and insurance guidelines will be followed during program implementation and will
maintain compliance with same and all Federal, State and relevant requirements.
Medical Qualifications and Education:
An M.D./D.O. who is in good standing, with an unrestricted license to practice medicine.
Possesses a fundamental knowledge of on-field medical emergency care (e.g. concussions,
cardiac emergencies, spinal injuries and heat-related illnesses).
Is trained in basic cardiopulmonary resuscitation, automated external defibrillator use and spine
boarding.
Has working knowledge of musculoskeletal injuries, medical conditions and psychological
issues affecting the athlete.
Has completed the state mandated concussion and sudden cardiac death modules.
Minimum of five years of successful experience as a medical practitioner in sports
medicine.
Knowledge of state law and regulation in area of school health practices (return to
Play protocol, cardiac screening and concussion management).
Provide evidence that a criminal record history check has been conducted and clearance
has been given by the Department of Education.
Available to cover or find coverage for all home and away Varsity Football games.

Scope of Services
Sideline Doctor for in-game attendance and preseason baseline testing
Attend all varsity football games, both home and away, as team doctor and performs all of the tasks related to
that position, including post-season play and will read the results of all post-concussive testing to determine
when an athlete may return to practice/competition, in accordance with West New York Board of Education
policy.
Response Requirements
All statements of qualifications for professional service contracts shall include at a minimum the following
information:
1. Names of individuals who will perform required tasks as well as the listing of their licenses
A. must at least be duly licensed in the state of New Jersey (NJ Med License), Impact Concussion Testing
certified with demonstrated track record for working with school districts in capacity of side-line doctor at
varsity games.
- in addition to 4-7 home games, 7-9 away games are anticipated to be scheduled (one per week each week
from early September through November - schedule will be shared with board-approved vendor)
B. Identify the person who will be primarily responsible for the services required by the Board of Education
and provide a description of the experience of the primary person with projects and issues similar to those
more specifically set forth in this proposal and on behalf of the Board of Education of West New York.
C. Identify person who will serve as back-up to the primary person including resumes of all parties.
2. Describe ability to provide services in a timely fashion including a description of your staffing and a
description of your familiarity with the services required by the Board of Education of West New York.
Evaluative Criteria
The evaluative criteria used in awarding a contract or agreement for professional services as described herein
shall include:
Qualifications of the individuals who will perform the tasks and the amounts of their
respective participation;
Experience and references (performance) of the respective therapist;
Ability to provide appropriate assessment and interventions in a timely manner; and
Cost competitiveness.

All RFQ's must be submitted no later than 11:00 a.m. on Thursday, May 28, 2026 at the Board of Education
