Formal bids will be received by Social Circle for furnishing all materials, labor, tools, equipment, and any other
miscellaneous items necessary for Walton Drive Emergency Repair and asphalt milling and resurfacing of all three streets

CONTRACTORS and SUBCONTRACTORS quoting on this Project will be required to comply with all Federal, State, and local
laws.
The City of Social Circle reserves the right to waive any informalities and to accept any Bid which in its opinion may be in
the best interest of Social Circle.
A bid bond in the amount of 5% of the total amount bid shall be required. In lieu of a bid bond, a cashier's check, cash
or an irrevocable letter of credit may be accepted. Both a performance and payment bond will be required in the amount
equal to 100% of the contract price. Proof of workers compensation and liability insurance will be required with the City
of Social Circle named as additional insured.

GENERAL CONDITIONS
STATEMENT OF EXPERIENCE AND QUALIFICATIONS:
The Contractor may be required, upon request, to prove to the satisfaction of the City of Social Circle that he/she has
the skill, experience, equipment and the necessary facilities and ample financial resources to perform the contract(s) in
a satisfactory manner and within the required time. If the available evidence of competency of any Contractor is not
deemed satisfactory, the bid may be rejected.
SUBCONTRACTS:
Contractor is specifically advised that any person, firm or other party to whom it is proposed to award a subcontract
must be acceptable to the City of Social Circle. All Subcontractors must submit a Non-Collusion Affidavit.
CHANGE ORDERS:
No oral statement of any person shall modify or otherwise change or affect the terms, conditions or specifications. All
change orders to the agreement will be made in writing and shall not be effective unless signed by an authorized
representative of the City of Social Circle.
PAVEMENT FINISH:
Milling and asphalt placement shall be performed as directed by the City of Social Circle. Final asphalt finish shall be
flush with and match the grade of the existing pavement surface, with a minimum 2% slope away from the centerline or
to match existing crown. The final work product shall provide a smooth road surface to safely accommodate traffic. Any
irregularities in asphalt material or surface finish shall be corrected by Contractor at no additional cost.
MEASUREMENT AND PAYMENT:
All labor, materials, equipment and items specified and incidental to the work shall be included in the unit price bids as
defined below. A 5% retainage will be withheld on all monthly pay requests until final project completion.
WALTON DRIVE EMERGENCY REPAIR
ITEM 1 - MILL ASPHALT PAVEMENT, 2 INCH DEPTH
The basis of payment for this item shall be square yard (SY); including all labor, equipment and materials required to
mechanically mill, remove, haul and dispose of asphalt pavement to be replaced as shown or indicated in the
CONTRACT DOCUMENTS.
ITEM 2 - ASPHALT PAVING - 12.5MM SUPERPAVE
The basis of payment for this item shall be per ton (TN); including all labor, equipment and materials required for
subgrade preparation, asphalt placement and compaction in accordance with Georgia DOT Standards.
ITEM 3 - CONCRETE DRIVEWAY CUT & REPAIR
The basis of payment for this item shall be square yard (SY) including all labor, equipment and materials required to
cut and repair driveway as shown or indicated in the CONTRACT DOCUMENTS.
ITEM 4 - CONCRETE SIDEWALK RESTORATION
The basis of payment for this item shall be square yard (SY) including all labor, equipment and materials required to
complete concrete sidewalks restoration to match the existing indicated in the CONTRACT DOCUMENTS.

ITEM 5 - PERMANENT SEEDING (DS3)
The basis of payment for this item shall be in linear foot (LF) including all labor, equipment and materials required
to complete the process of permanent seeding for site.
ITEM 6 - SOD RESTORATION (DS4)
The basis of payment for this item shall be in square foot (SF) including all labor, equipment and materials required
to complete the process of sod restoration to match existing present before emergency repair for site.
ITEM 7 - CONCRETE CURB & GUTTER REPLACEMENT
The basis of payment for this item shall be per linear foot, by type, including subgrade preparation, formwork,
finishing, expansion joints, and all incidental labor, equipment, and materials required for installation to match
existing as shown or indicated in the CONTRACT DOCUMENTS.
ITEM 8 - TRAFFIC CONTROL
The basis of payment for this item shall be per lump sum (LS); including all labor, materials, equipment required to
provide work zone traffic control including signage, barrels, cones, flagmen, and pedestrian safety measures in
accordance with Georgia DOT Standards. Any proposed temporary road closures or detours must be approved in
advance by Social Circle.

PROJECT SPECIFICATIONS AND INSTRUCTIONS
Scope of Work:
Walton Drive Emergency Repair: The Contractor shall provide all labor, equipment, materials and any other
miscellaneous items necessary for asphalt milling to a depth of 2", traffic control, cleanup, surface prep, tack coat,
12.5mm asphalt resurfacing, concrete driveway repair, site restoration.
Corby Lane, Justin Court, Emily Lane Milling and Asphalt Replacement: The Contractor shall provide all labor, equipment,
materials and any other miscellaneous items necessary for asphalt milling to a depth of 2", asphalt removal, hauling,
traffic control, cleanup, surface prep, tack coat, 12.5mm asphalt resurfacing/replacement with 2" compacted depth per
Georgia DOT Standards for all streets within Corby Creek Subdivision.
Project Management/Inspections: Project management and inspection will be provided by the Social Circle Public Works
Department and/or Precision Planning, Inc.
Schedule: All work shall be completed within 45 calendar days from the date of Notice to Proceed. Prior to commencing
the work, the Contractor shall provide a written schedule to Social Circle Public Works regarding the sequence/order of
the work. The Contract shall include a provision for liquidated damages to be assessed at the rate of $500 per calendar
day for late completion. This is a residential neighborhood and work shall be done from 8:00 am - 6:00 pm.
Traffic Control: Contractor shall be responsible for Traffic Control, in accordance with Georgia DOT Standards, and shall
provide all signage, personnel and miscellaneous equipment to maintain alternating single lane traffic while work is being
conducted and normal two lane traffic when personnel are not on site. The Social Circle Public Works Department must
approve any proposed temporary road closures under this contract, and if approved, the Contractor shall be responsible
for establishing required signage and maintaining acceptable detour routes during the construction period.
Safety: Contractor shall be responsible for jobsite safety, and the safety of the traveling and general public within the
work zone.
Access/Cleanup/Disposal: Contractor shall be responsible for coordinating access to private property during the
execution of the work, and for cleanup of any millings or debris resulting from the project on adjacent private property
or within public right-of-way. The Contractor shall remove, haul and dispose of all asphalt millings at an approved facility.
