Vendor Service Contract for Emergency Medical Services (EMS) Billing and Data Collection Services

For this RFP, the City of Milwaukee is using a Bonfire portal for accepting and evaluating proposals digitally.

All required documents are available for download on the Bonfire portal.

Your submission must be uploaded, submitted, and finalized prior to the Closing Time of November 12, 2024 at 4:30 PM CT. We strongly recommend that you give yourself sufficient time and at least ONE (1) hour before Closing Time to begin the uploading process and to finalize your submission.

Important Notes:

This RFP includes an 18% SBE requirement. Bidder must submit a completed copy of OEI Form A with their bid submittal.

The City's Living Wage Ordinance applies to this RFP. The "Affidavit of Compliance with Living Wage Provision" must be submitted with the RFP.

The awarded Contractor will be required to submit proof of Insurance. Please refer to the Insurance Requirements section on the Invitation to Bid form.
