swing back, radial elbow construction for increased mobility and comfort, rope

The vendor will be held to the requirements of F.O.B. Destination as set forth in the

Commonwealth of Pennsylvania Terms and Conditions. The vendor may quote shipping

costs, but it must be listed on a separate line item in the quote. The awarded vendor will

be paid for the total line items on the quote.

Additional Requirements:

• Measurements to be completed by awarded vendor once order is placed. Please enter the cost

for standard size. Please notate on quote the cost per unit for larger sizes if pricing is different

than standard sizing. Order can be adjusted to reflect higher cost if larger sizes are required.

o A date/time will be agreed upon by the awarded vendor and the end user once order is

awarded.

• Please provide estimated shipping lead-time.

• Complete and attach GSPUR-89 – Reciprocal Limitations Act Requirements (Required for orders

exceeding $10,000.00) with bid.
