This bid sheet must accompany the proposal. Any alterations or changes to this document will deem
that bid unresponsive and will no longer be given further consideration.
Sealed bids will be received until 4:30 P.M., on July 1, 2026, at the Glades County Community
Development 198 Sixth Street SW, Moore Haven, FL 33471 and opened immediately thereafter at the
same address. The work to be performed at the property identified below, includes furnishing all labor,
materials, supplies, tools, equipment and services required and necessary to perform and complete the
construction work described herein and more specifically identified in the attached Rehabilitation
Specifications Analysis Report. Said work to be done strictly in accord with GCCD's rehabilitation
policies and procedures. The construction work will be performed at the property owned by and
located at

THE WORK WRITE-UP/BID FORM IS A GENERAL OUTLINE OF THE WORK TO BE PERFORMED. PLEASE
REFER TO THE PROJECT SPECIFICATION BOOKLET FOR DETAILED INSTRUCTION FOR EACH ITEM NOTED
BELOW. Do not include labor amount per item in your figures below, Labor is now a separate item. Also,
when you are able to indicate the name of the product and or manufacturer, such as air conditioner,
Rheem or Goodman.

1. REMOVE AND REPLACE CARPET FLOORING
• Remove and dispose of existing worn and mold-affected carpet
and padding throughout designated interior living areas.
• Inspect subfloor for moisture damage; notify SHIP Administrator of
any conditions requiring additional repair prior to installation.
• Furnish and install new floor covering suitable for residential use,
including:
o Moisture-resistant flooring material (e.g., vinyl plank or
comparable SHIP-approved finish)
o Appropriate underlayment (if required)
o All trims and transitions
o Installation shall be level, secure, and free of defects.

2. INTERIOR DRYWALL REPAIRS
Patch and repair damaged interior drywall at various locations
throughout the home.
Scope includes:
o Removal of damaged drywall sections

o Installation of new drywall as needed
o Tape, float, sand, and feather repairs to blend with adjacent
surfaces
Repair areas to be finished ready for paint (paint by others
unless noted otherwise).

3. BATHROOM TUB REPLACEMENT – TWO (2) LOCATIONS
Remove and dispose of two (2) existing bathtubs.
Furnish and install two (2) walk-in shower systems, one in each bathroom,
including:
o Shower base or pan
o Shower wall panels or approved finish
o Entry door or curtain configuration as approved
o Necessary plumbing reconnections
All installations shall be water-tight and properly sealed.
Coordinate inspections as required.

4. WATER CLOSET REPLACEMENT
Remove and dispose of one (1) existing water closet (toilet).
Furnish and install one (1) new residential water-efficient water closet.
Include new wax ring, supply line, and secure installation per plumbing
code.
Verify proper flushing and no leaks.

5. KITCHEN CABINET REPLACEMENT
Remove and dispose of existing kitchen cabinets.
Furnish and install new kitchen cabinets in the same layout and
configuration as existing, including:
o Base cabinets
o Wall cabinets
o All necessary hardware
Cabinets shall be securely anchored, level, and plumb.
Contractor to coordinate with existing countertops for proper fit
(countertops not replaced unless noted)

Work is to be performed in a workmanlike manner, in accordance with the Standard Rehabilitation Specifications,
local codes and manufactured specifications.
The contractor shall be responsible for repairs and/or reinstallations of materials/equipment/fixtures damaged or
removed due to any work item contained herein.
Contractors shall properly dispose of all fixture, materials and other items removed from the dwelling unless
otherwise specified herein.
ALL ITEMS MUST BE COST ITEMIZED IN THE SPACE PROVIDED OR THE BID WILL BE REJECTED. ALL ADDENDUMS
MUST BE SIGNED AND SUBMITTED WITH THIS BID.
Work must be completed and approved within 90 days of issuance of the NOTICE TO PROCEED.
I hereby certify that I am a licensed by the State of Florida, Department of Business and Professional Regulation
and that I am eligible to participate in Glades County housing Programs. I also agree that change orders above
the original contract amount shall only be paid for with housing program funds to correct documented code
violations or to meet Section 8 housing Quality Standards. Change orders must be approved by the homeowner
or his or her representative, the contractor and local government prior to any initiation of work based on that
change order.
