This bid sheet must accompany the proposal. Any alterations or changes to this document will deem
that bid unresponsive and will no longer be given further consideration.
Sealed bids will be received until 4:30 P.M., on July 6, 2026, at the Glades County Community
Development 198 Sixth Street SW, Moore Haven, FL 33471 and opened immediately thereafter at the
same address. The work to be performed at the property identified below, includes furnishing all labor,
materials, supplies, tools, equipment and services required and necessary to perform and complete the
construction work described herein and more specifically identified in the attached Rehabilitation
Specifications Analysis Report. Said work to be done strictly in accord with GCCD's rehabilitation
policies and procedures. The construction work will be performed at the property owned by and
located at

THE WORK WRITE-UP/BID FORM IS A GENERAL OUTLINE OF THE WORK TO BE PERFORMED. PLEASE
REFER TO THE PROJECT SPECIFICATION BOOKLET FOR DETAILED INSTRUCTION FOR EACH ITEM NOTED
BELOW. Do not include labor amount per item in your figures below, Labor is now a separate item. Also,
when you are able to indicate the name of the product and or manufacturer, such as air conditioner,
Rheem or Goodman.

1. MASTER BATH - BATHTUB SHOWER CONVERSION ADA COMPLIANT ROLL
IN SHOWER
Install a glazed porcelain bath set comprised of a tub soap dish, 24" towel
bar. Owner's choice of stock color.

2. MASTER BATH – BLOCKING FOR GRAB BARS
Install Grab Bars for tub/shower area for framed walls using min. 2"x6"
pressure treated blocking or 1/2" plywood and for CMU walls using min.
1"x6" pressure treated wood blocking at desired height to meet
accessibility requirements.
Furnish and install grab bars withing the tub/shower area per the current
Florida Building Code. Horizontal and vertical grab bars shall be installed
per the Building Code of Jurisdiction. Each grab bar shall be of proper
length and located in the proper positions as determined by the Building
Code of Jurisdiction. All bars shall be round with a minimum of 1.25"
diameter, maximum 2" diameter, type 304, 18 gauge (.049), brushed satin
stainless steel, with round snap on covers and welded 3" flanges and a 1.5"
space between the bar and wall. Bars must be fastened securely to the
wall framing.

3. MASTER BATH WALL TILE - SHOWER SURROUND
Remove existing shower tile, tile backer, pan and water damaged wall
materials affected by framing of the new shower. Replace damaged
studs and furring strips. Shower pan to be installed per Spec 5425.
Install wall tile of 12"x24"x1/4" polished porcelain wall tile or approved
equivalent to ceiling over 1/2"-5/8" cement tile backer board equal to
Wonder board, or Hardie Backer board, or Dens Shield, or approved
equivalent. Place wood backing/blocking per Spec 5414 for handicap
bars. Install a new washer less single handle diverter assembly faucet
valve with screwdriver stops, American Standard "Town Square" or
approved equivalent. Max. 2.0 gpm water saving shower head. Include
preformed base, cap, stop, return, and trimmer pieces to complete
installation up to ceiling height. After at least 24 hours drying time, apply
Spectra LOCK® PRO Grout. Clean floor and apply mildew resistant white
Low VOC silicone caulk to all edge seams and pipe penetrations. Wall
tile allowance $2.50/sq/ft. Ceramic Tile Baseboard. Install a Ceramic Tile
Baseboard using same size as floor tile and cut into 4" to 6" wide strips
over a sound surface with troweled adhesive. After at least 24 hours
drying time, apply latex based Portland cement grout. Clean all excess
grout and apply mildew resistant white silicone caulk at all seams. PREP
& PAINT ROOM w/ PAINTED TRIM-LOW VOC Using lead safe work
practices remove & dispose of all loose material & dust prior to
installation of new materials. All cracked or loose plaster is to be
repaired with a bedding coat of Durabond & fiberglass mesh tape. If
plaster & lath boards are loose, resecure or remove & replace it with
drywall patch. Sanding of any surfaces contacting or adjoining a lead-
based painted surface shall be done with appropriate procedures such
as using a HEPA filtered sanding vacuum or a wet sanding method.
Prime as necessary to seal stains, raw plaster, etc. Paint ceilings two
coats in flat ceiling white & walls in eggshell or satin finish cut-in neatly
to trim & at all corners & edges. Prep trim doors and windows by de-
glossing painted trim prior to painting and prep all doors to be painted.
Apply two coats of latex semi-gloss paint to cover completely &
uniformly. Colors are the choice of the owner from stock colors.
Maximum of 2 color choices for interior. All paints and primers must not
exceed the following maximum VOC requirements: Flats 50 g/L; Non-
flats 50 g/L; Floor 100 g/L; Anti-corrosive 250 g/L. All adhesives must
comply with Rule 1168 of the South Coast Air Quality Management
District.www.aqmd.gov/rules/reg/reg11/r1168.pdf All caulks and
sealants must comply with Regulation 8, Rule 51, of the Bay Area Air
Quality Management District (BAAQMD).

4. MASTER BATH - HANDHELD SHOWER DIVERTER W/ ADJUSTABLE RAIL
Install a new washer less single handle diverter handheld assembly faucet
valve with screwdriver stops, Moen "Chateau" or equal.

5. IMPACT WINDOW
A. GENERAL INFORMATION
1. Replace windows with new single hung aluminum or Vinyl impact
windows that meet the current Florida Building Code and current
applicable code requirements for product approval. All provided
windows must meet the required wind rating. All existing openings to be

examined for attachment requirements per manufacturer's
requirements and Building Code of Jurisdiction. New windows are to be
of similar size to existing windows. ALL FENESTRATION MUST COMPLY
WITH THE FLORIDA ENERGY CODE.
B. PROJECT CONDITIONS
1. Remove and dispose of all existing windows to be replaced; clear
room and/or cover all surfaces prior to beginning work; remove and
dispose of properly including: all damaged drywall, insulation, and
framing. Where security system wiring exists, the Contractor shall
preserve the security system as it pertains to the windows. This may
include replacing contacts and repairing the security system circuitry
affected by replacing the windows.
C. PRODUCTS
1. Impact Windows to be PGT, CGI, or approved equal, single hung
aluminum or Vinyl windows with insect screens.
2. All Bathroom windows to be obscure glass.
3. Color selection may be limited due to supply availability and/or time
constraints required to complete the construction.
D. INSTALLATION
1. Install all windows per manufacturer's requirements, NOAs, and
Building Code of Jurisdiction and form a watertight installation with drip
at head. Seal all joints.
Repair all exterior/interior damage to match existing as best as possible. $
6. HOT WATER HEATER RELOCATE TO EXTERIOR
A. Provide all labor, materials, equipment, permits, inspections, and
incidentals necessary to disconnect, remove, relocate, and reinstall
the existing domestic hot water heater from its current interior
location to an approved exterior location, in full compliance with
applicable Florida Building Code (FBC), Florida Plumbing Code (FPC),
Florida Fuel Gas Code (if applicable), Florida Mechanical Code,
Florida Energy Conservation Code, and NEC requirements.
B. Safely remove existing unit without damage to surrounding walls,
floors, or finishes.
C. Cap all abandoned plumbing, gas, and electrical connections in
accordance with applicable code.
D. Patch and restore interior finishes as required to maintain habitable
condition.
E. Install the water heater at an approved exterior location compliant
with: FPC Section 504 FBC Chapter 3 (Protection of openings and
building envelope). Water heater shall be :
1. Installed on a rated exterior concrete pad or elevated corrosion-
resistant stand Properly anchored for wind load resistance per
FBC Residential Chapter 3A minimum of 1 inch above grade (or
as required by manufacturer)Clearance requirements shall
comply with: Manufacturer’s installation instructions FPC Section
504.3Provide an exterior-rated.
2. Insulated water heater enclosure, or listed water heater rated
for outdoor installation. Tank, piping, and fasteners shall be
corrosion-resistant or protected against weather exposure per
FBC Section 305.All penetrations through exterior walls shall be

sealed watertight with approved materials.
3. Reconnect cold and hot water piping with approved piping
materials per FPC Chapter 6Dielectric unions where dissimilar
metals are used.
4. Install a shutoff valve on the cold-water supply line per FPC
Section606.2.
5. Install a temperature and pressure relief valve with discharge
piping: Same diameter as valve outlet Terminating 6–24 inches
above grade No threaded end Compliant with FPC Section 504.6.
6. Insulate exposed hot water piping per Florida Energy
Conservation Code requirements.

Work is to be performed in a workmanlike manner, in accordance with the Standard Rehabilitation Specifications,
local codes and manufactured specifications.
The contractor shall be responsible for repairs and/or reinstallations of materials/equipment/fixtures damaged or
removed due to any work item contained herein.
Contractors shall properly dispose of all fixture, materials and other items removed from the dwelling unless
otherwise specified herein.
ALL ITEMS MUST BE COST ITEMIZED IN THE SPACE PROVIDED OR THE BID WILL BE REJECTED. ALL ADDENDUMS
MUST BE SIGNED AND SUBMITTED WITH THIS BID.
Work must be completed and approved within 90 days of issuance of the NOTICE TO PROCEED.
I hereby certify that I am a licensed by the State of Florida, Department of Business and Professional Regulation
and that I am eligible to participate in Glades County housing Programs. I also agree that change orders above
the original contract amount shall only be paid for with housing program funds to correct documented code
violations or to meet Section 8 housing Quality Standards. Change orders must be approved by the homeowner
or his or her representative, the contractor and local government prior to any initiation of work based on that
change order.
