Event Description
The Office of Community Engagement (OCE) within South Dakota's Division of Family and Community Health is seeking proposals from qualified organizations to support Community Health Workers (CHWs) and Community Health Representatives (CHRs) and the profession in South Dakota. The selected organization will provide ongoing technical assistance and support to CHWs, CHRs, and the organizations that employ them, provide support and guidance for organizations who are looking to start a program, assist with South Dakota (SD) Medicaid enrollment and reimbursement, and assist OCE in coordinating efforts and activities to support programming focused on the prevention and management of chronic disease (opioid, immunization, heart disease and stroke prevention, diabetes, etc.). The selected organization will provide services that are consistent with the programs’ goals, which reflect evidence-based approaches and are recommended by the Centers for Disease Control and Prevention (CDC).
Offerors may email inquiries concerning this RFP to obtain clarification of requirements. No inquiries will be accepted after the date and time indicated in the Schedule of Activities. Inquiries must be emailed to Corryn Gabbert, Community Engagement Coordinator, at Corryn.gabbert@state.sd.us with the subject line “RFP #25-0904006-009”.
The State will to respond to offeror’s inquiries (if required) via e-mail. In addition, all inquiries and the State’s response will be posted on the state’s e-procurement system. condition of this RFP. Offerors will be notified in the same manner as indicated above regarding any modifications to this RFP.
