Supply & Inscribe Medal of Honor Headstones and Markers for MPS
The purpose of this amendment is to address submitted questions and to provide a revised combined synopsis/solicitation and Statement of Work.

Note that the response due date and time is NOT extended and remains 23 July 2025 at 10:00 a.m. CST.

The Statement of Work has been revised to update the phone number in Section 6.1 and to add information regarding the required Aging Report in Section 7.2. See attached revised SOW.

The combined synopsis/solicitation is revised to provide clarification to the required sample sizes and to correct minor grammatical errors.

Q: On the Medal of Honor solicitation, on page 4 of the Combined Synopsis/ Solicitation Notice it says for samples, to send a marble sample 13" x 42" x 4" and a granite sample 12" x 24" x 4". Then under Sample Requirements: it clearly states "Full size samples will NOT be accepted and will be refused at delivery". The sizes you requested are FULL SIZE samples. Are they going to be accepted or do you really want the 3/4" thick samples that we have been sending for many years? For the headstones, we normally send samples that are 13" x 24" x 3/4" and on the markers 12" x 24" x 3/4".

A: The combined synopsis/solicitation has been revised to reflect the required sample size as follows:
Granite Sample: Width - 24 in., Thickness - 3/4 in. and Height- 12 in.
Marble Sample: Width - 13 inches, Thickness - 3/4 in. and Height - 24 in.
