The City of Temple Terrace ("City") is soliciting proposals from qualified Vendors to provide storm
debris collection, hauling, reduction, and lawful disposal services. This contract will ensure timely,
efficient, and compliant removal of storm-generated debris following hurricanes, tropical storms,
tornados, or other declared emergencies within City limits. This contract is a three (3) year
continuing contract with the option of up to two (2) additional one (1) year extensions.
The City of Temple Terrace issues its solicitations electronically through its contracted vendor,
Onvia DemandStar as well as the City's website. All addenda to such solicitations will be issued
through both Onvia DemandStar and the City's website. However, if you have obtained this
solicitation from a source other than Onvia DemandStar, such as the City's website which does
not track downloads, you will not be listed on the plan holders list and may not receive notification
of any addenda issued. Failure to receive any issued addenda shall not relieve the bidder/proposer

II. DEFINITIONS
The terms used in this RFP, unless the context clearly requires a different construction and
interpretation, have the following meanings:
a. City: The City of Temple of Terrace in Hillsborough County, Florida.
b. Contract: The written Contract between the City and the successful Vendor, that results
from this RFP to include its attachments, modifications/amendments, addenda and/or
renewals.
c. Contractor: The organization or individual providing services to the City, in accordance
with the terms of any Contract which may result from this RFP.
d. Day: A calendar day, unless otherwise noted.
e. Proposal: A Vendor's response to this Request for Proposal.
f. Responsible Vendor: A Vendor who has the capability in all respects to fully perform
the Contract requirements and the integrity and reliability that will assure good faith
performance.
g. Responsive Proposal: A Proposal submitted by a Responsible Vendor, which conforms
to all material aspects of this RFP.
h. State: State of Florida.
i. Vendor: A legally qualified corporation, partnership, or other business entity submitting
a Proposal to the City in response to this RFP.
III. SCOPE OF SERVICES
1. Purpose
The City of Temple Terrace ("City") is soliciting proposals from qualified Vendors to provide
storm debris collection, hauling, reduction, and lawful disposal services. This contract will
ensure timely, efficient, and compliant removal of storm-generated debris following
hurricanes, tropical storms, tornados, or other declared emergencies within City limits.
2. Background
The City of Temple Terrace, located in Hillsborough County, Florida, is vulnerable to seasonal
storms and extreme weather events that produce large volumes of vegetative debris,
construction and demolition debris, white goods, and other storm-related waste. The City
seeks pre-qualified Vendors support to assist City staff in clearing roadways, rights-of-way,
and public property to ensure public safety and compliance with FEMA and State guidelines.
3. Scope of Services
The Contractor shall provide, at minimum, the following services:
A. Debris Removal and Collection
a. Perform curbside collection of storm-related debris from public rights-of-
way and designated areas within the City.
b. Separate and collect debris into categories, including but not limited to:
i. Vegetative debris (tree limbs, branches, stumps, etc.),
ii. Construction and Demolition (C&D) debris,
iii. White goods (appliances),
iv. Hazardous waste (handled per federal/state regulations),
v. Electronic waste,
vi. Soil, Mud, and Sand,
vii. Putrescent Debris, and
viii. Chemical, Biological, Radiological, and Nuclear Contaminated
Debris.
c. Ensure access to critical public infrastructure (roads, drainage systems,
utilities).
d. Create a plan to stabilize, remove, and dispose of vehicles, boats, and
debris from public waterways.
e. Create a plan to stabilize, remove, store, dispose of vehicles to include
passenger and commercial to include battery powered vehicles.
B. Debris Hauling and Disposal
a. Transport collected debris to City-designated Temporary Debris Staging
and Reduction Sites (TDSRS) or approved disposal/reduction facilities.
b. Provide reduction methods (grinding, chipping, burning, cutting, if
approved by regulatory agencies).
c. Dispose of debris at licensed facilities, ensuring documentation of
tonnage and load tickets.
C. Monitoring and Documentation
a. Provide daily operational reports, including quantities collected, locations
serviced, and disposal tickets.
b. Maintain all documentation required for FEMA Public Assistance
reimbursement, including load tickets, haul routes, and
Contractor/Subcontractor certifications.
c. Cooperate with the City's debris monitoring Contractor or designated
staff.
D. Equipment and Staffing Requirements
a. Furnish all necessary labor, equipment, trucks, trailers, loaders, and
support staff.
b. Maintain sufficient standby resources to mobilize within 24-48 hours of
notice-to-proceed.
c. Ensure all equipment is clearly labeled with Contractor identification.
E. Health, Safety, and Environmental Compliance
a. Conduct all operations in compliance with OSHA, EPA, FDOT, and Florida
Department of Environmental Protection regulations.
b. Implement traffic control and safety measures in collection and hauling
areas.
c. Ensure hazardous materials are segregated and handled properly.
4. Contractor Qualifications
Proposers must demonstrate:
a. Prior experience with storm debris removal and FEMA reimbursement
compliance.
b. Availability of resources to handle large-scale debris volumes.
c. Ability to mobilize crews and equipment quickly.
d. Adequate insurance and bonding capacity.
5. Performance Standards
a. Mobilization must occur within 24 - 48 hours of notice from the City.
b. Collection must be conducted systematically across all affected areas until
cleared.
c. All load tickets and disposal records must be submitted daily.
d. Contractor must maintain clear communication with the City's Emergency
Operations Center (EOC).
6. Contract Term
This will be an as-needed/standby contract activated upon a declared emergency or City
directive. The term shall be for three (3) years, with the option to renew for up to two (2)
additional one-year terms upon mutual agreement.
7. Payment and Invoicing
Payment will be based on unit prices (per cubic yard, per ton, per haul load, etc.) as listed in
the Contractor's Proposal.
No payment will be made without proper load tickets and supporting documentation.
Contractor must submit invoices within thirty (30) days of completed work for City review
and approval.
8. Compliance with FEMA/State Requirements
The Contractor shall perform all services in strict compliance with FEMA Public Assistance
Program and Policy Guide (PAPPG), Stafford Act requirements, and State of Florida
Emergency Debris Management regulations to ensure maximum cost recovery for the City.
IV. INSTRUCTIONS TO PROPOSERS
Storm Debris Collection and Disposal - Two Levels of Activation
1. General
The City of Temple Terrace ("City") is soliciting sealed Proposals from qualified Vendors to
provide storm debris collection, hauling, reduction, and lawful disposal services under a as-
needed contract. The City reserves the right to activate services at two levels of mobilization
depending on the severity of events:
* Level 1 Activation - Localized Events ( 10,000 cubic yards)
* Level 2 Activation - Major Disaster Events (> 10,000 cubic yards; typically
federally/state-declared disasters)
Vendors must submit pricing for both levels of activation as provided in the Proposal
Pricing Sheet.
2. Submission of Proposal
Proposals must be submitted in a sealed envelope/package, clearly marked:

3. Required Proposal Forms
Each Proposal must include the following completed and signed documents:
1. Bid Proposal and Instructions to Bidders, and Pricing Sheet (Section B)
2. List of Subcontractors and Equipment (Section C, and Section D)
3. Proof of Insurance & Bonding Capacity (Provide Proof of Insurance, and fill out
Section E for Bid Bond)
4. Anti-Human Trafficking Affidavit (Section F)
5. Non-collusion Affidavit (Section G)
6. Public Entity Crimes Certifications (Section H)
7. Contractor and Subcontractor E-Verify Affidavit (Section I)
8. References and Project Experience (demonstrating FEMA-compliant debris
removal experience, provide at least 3 references including project/contract date,
contact name & phone numbers)
9. Acknowledgment of Addenda (if applicable)
Failure to submit all required forms in its entirety may result in your Proposal being
rejected. Please indicate N/A if answers to any questions listed above is irrelevant to you to
show they were not overlooked.
4. Basis of Award
The City intends to award a contract to the qualified Proposer(s) meeting all
requirements. Consideration will be given to:
* Unit prices submitted on Proposal Pricing Sheet
* Demonstrated experience with FEMA-reimbursable debris operations
* Availability of personnel, equipment, and resources
* Compliance with RFP specifications
The City reserves the right to award to multiple Vendors if deemed in the City's best
interest.
5. Pricing Requirements
* All unit prices shall be fully burdened and include all labor, materials, equipment,
fuel, overhead, profit, and incidentals.
* Pricing must be submitted for both Level 1 and Level 2 activations.
* Contractor shall honor Proposal pricing for the entire contract term.
* No separate mobilization fees shall be paid unless specifically listed on the
Proposal Pricing Sheet.
6. FEMA Compliance & Documentation
Contractor services must comply with FEMA's Public Assistance Program and Policy
Guide (PAPPG) to ensure the City's eligibility for federal reimbursement. Requirements
include, but are not limited to:
* Use of load tickets with cubic yard/tonnage measurements, truck certifications, and
haul routes.
* Daily activity logs and GPS tracking of vehicles (if available).
* Proper segregation of debris categories (vegetative, C&D, white goods, hazardous
waste, electronic waste).
* Compliance with Stafford Act, Florida DEP, Florida DOT, Florida Division of
Emergency Management, and OSHA requirements.
* Cooperation with the City's debris monitoring contractor or designated staff.
Failure to comply with FEMA requirements may result in non-payment.
7. Insurance & Bonds
The selected Contractor shall provide and maintain, at its own expense:
* General Liability Insurance: $3,000,000 per occurrence / $5,000,000 aggregate
* Automobile Liability Insurance: $1,000,000 combined single limit
* Workers' Compensation: Statutory requirements
