Description

The Indian Health Service, Shiprock Service Unit, located in/at Hwy 491 N, Shiprock, New Mexico, 87420, has a requirement for a Case Manager Consultant. The consultant shall provide technical assistance to assess and improve the Case Management Program across three healthcare facilities: Northern Navajo Medical Center, Dzilth-Na-O-Dith-Hle Health Center, and Four Corners Regional Health Center.
