Notes	IF YOU ARE ALREADY AN LISD BOARD AWARDED VENDOR ON RFP #2832-21, RFP #2832A-21, RFP #2832B-21, RFP #2832C-21 or RFP #2832D-21 YOU DO NOT NEED TO RESPOND TO THIS RFP. Lewisville Independent School District ("LISD, "District") is accepting proposals for qualified persons and/or companies to provide various Special Education Contracted Services. These services shall be purchased on an "As Needed" basis, with LISD making the sole determination of need. Multiple provider awards are anticipated to provide LISD with a variety of resources for needed services. Services requested include, but not limited to: evaluation, consultation, and direct services. All interested vendors, including sole source providers, are encouraged to respond to this request for proposal in its entirety and should submit all required bid attachments regardless of sole source status to be considered. Missing or incomplete information will impact the evaluation criteria scoring. RFP #2832E-21 Special Education Contracted Service shall be awarded at District discretion, to multiple Proposers supplying comparable services, also known as a multiple award schedule, or award the Contract to a single Proposer. The District's decision to make multiple awards or a single award will be based upon the District's sole discretion regarding the type of award that provides best value to the District. This supplemental request for proposal is being issued in accordance with Title 2 of the Code of Federal Regulations (2 CFR) Part 200 requirements to procure vendors who provide services purchased by LISD using federal dollars. This proposal may also be used with local, state, federal and grant funded programs. The anticipated term of this proposal, if awarded, shall be from May 2025 - December 2026.
