SOQ ERO DO #02-F-2-24/25 - Children and Youth Targeted Case Management

The Carroll County Bureau of Prevention, Wellness, and Recovery; Local Behavioral Health Authority (LBHA) desires to identify vendors to provide Mental Health Case Management: Care Coordination for Children and Youth, which includes young adults up to age 21 for Carroll County. Mental Health Case Management: Care Coordination for Children and Youth allows for a multi-level continuum of care coordination using wraparound principles. This multi-level continuum of care provides care coordination to children and youth to support a transition back to a home environment, remain in their home or current living arrangement, move to a lower intensity of services or restrictiveness of placement, or otherwise maintain and improve functioning and well-being.
