REQUEST FOR SEALED QUOTATIONS
Sealed Quotations will be received on September 10, 2026 by the Terrebonne Parish Consolidated
Government (TPCG), Purchasing Division, located at 301 Plant Road, Houma, Louisiana, 70363, until
2:00 p.m. as shown on the Purchasing Division Conference Room Clock, all quotations will be opened
and read aloud. Subsequent to the opening, the quotes will be evaluated by the appropriate parties
and submitted to the proper authority with a recommendation for awarding, rejecting, or holding for
further evaluation.
No quotation received after the scheduled time for opening will be considered. Failure of the United
States Postal Service (USPS) to deliver the quotations in a timely manner shall not be considered due
cause for the scheduled time of the opening to be extended. Quotations must be submitted on the
form furnished with the Quotation Documents.

This Scope of Work shall require a Louisiana Contractors license number for Building Construction.
The envelope shall be marked with the quotation title, name, address, and license number of the
Contractor as set forth in this document.
The Terrebonne Parish Consolidated Government reserves the right to reject any quotation for just
cause in the best interest of the Terrebonne Parish Consolidated Government.

INSTRUCTIONS AND GENERAL CONDITIONS
The project is being funded through Community Development Block Grant Disaster Recovery (CDBG-
DR) funds the Resilient Communities Infrastructure Program (RCIP), granted to TPCG by the Louisiana
Office of Community Development-Disaster Recovery. The CBDG-DR Rider attached hereafter is
required to be executed in full by the awarded contractor.
Terrebonne Parish Consolidated Government reserves the right to award contracts as shall in its
judgment be in the best interest of Terrebonne Parish Consolidated Government. Every contract or
order shall be awarded to the lowest responsive and responsible Contractor, taking into consideration
the conformity with the specifications.
All quoted prices shall remain valid for forty-five (45) days. Terrebonne Parish Consolidated
Government and the lowest responsive and responsible Contractor by mutual written consent may
mutually agree to extend the deadline for award by one (1) or more extensions of thirty (30) calendar
days.
**SIGNATURE AUTHORIZATION:
Written evidence of the person signing the quotation SHALL be submitted at the time of submittal,
in accordance with LA R.S. 38:2212(B)(5) as follows:
(a) The signature on the quotation is that of any corporate officer listed on the most
current annual report on file with the secretary of state, or the signature on the
quotation is that of any member of a partnership, limited liability company, limited
liability partnership, or other legal entity listed in the most current business records on
file with the secretary of state.
(b) The signature on the quotation is that of an authorized representative as
documented by the legal entity certifying the authority of the person.
(c) The legal entity has filed in the appropriate records of the secretary of state of this
state an affidavit, resolution, or other acknowledged or authentic document indicating
the names of all parties authorized to submit quotations for public contracts. Such
document on file with the secretary of state shall remain in effect and shall be binding
upon the principal until specifically rescinded and canceled from the records of the
office.
Failure to include the appropriate signature authorization shall result in rejection of the
quotation as non-responsive.
PREPARATION AND SUBMISSION OF QUOTATIONS - Quotations shall be submitted at the time and
place indicated in the "Request for Sealed Quotations" and shall be enclosed in an opaque sealed
envelope. If the quotation is sent through the mail or other delivery system, the sealed envelope
should be enclosed in a separate envelope with the notation "SEALED QUOTATION ENVELOPE
ENCLOSED" on the face of it.

A vendor may alter or correct an entry on the quotation form by crossing out the entry, entering the
new figure above or below the deleted entry, and initialing on the line of change. The crossing out of
and initials shall be legibly handwritten with ink or typed. Any ambiguity arising from entries altered or
corrected on the Quotation Form will cause the rejection of said quotation as non-responsive.
All vendors who submit more than one quotation for the same work from an individual, firm or
partnership, a corporation or association under the same or different name will have their quotation
rejected. Reasonable grounds for believing that any vendor has interest in more than one quotation
submitted for the same RFQ will cause the rejection of all such quotations in which the vendor is
interested. If there are reasonable grounds for believing that collusion exists among vendors, the
quotation of participants in such collusion will not be considered.
QUOTATION OPENING - All vendors may attend the quotation opening but no information or opinions
concerning the ultimate quote award will be given at the opening or during the evaluation process.
Actual quotation documents will not be available until after the award of the quotation. Anyone who
makes such a request must do a "Request for Public Record." However, quotation tabulations may be
examined 72 hours after a request is made.
TIME FOR RECEIVING QUOTATIONS: Sealed quotations shall be either hand delivered by the vendor or
his agent in which instance the deliverer shall be handed a written receipt, or such quotations shall be
sent by registered or certified mail with a return request. Quotations may be delivered by a package or
express delivery service or carrier. However, if this form of delivery is utilized, the form of delivery
receipt utilized by said carrier or service shall be considered as sufficient written receipt.
CANCELATION OF CONTRACT:
The TPCG has the right to cancel any contract for cause, including but not limited to, the following:
failure to deliver within the time specified in the contract; failure of the product or service to meet
specifications, conform to sample quality or to be delivered in good condition; misrepresentation by
the contractor; fraud, collusion, conspiracy or other unlawful means of obtaining any contract with the
TPCG; conflict of contract provisions with constitutional or statutory provisions of state or federal law;
any other breach of contract.
CONTRACT TIME:
The work will be completed within 50 calendar days after receipt of order
LIQUIDATED DAMAGES:
By submission of quotation, Contractor unequivocally accepts the liquidated damages provisions set
out herein in the event of failure, neglect or refusal to complete the work, or any designated part of
the work, within the corresponding contract times.
By submission of quotation, Contractor agrees that for each calendar day beyond the Contract Time
set forth in the Agreement, and extension thereof, that the Work remains incomplete, the Owner
may retain from the total amount of the Contract Price, as Liquidated Damages the following sums:
Two Hundred Fifty Dollars ($250.00) a day.

The continuation of the contract shall be contingent upon the appropriation of funds to fulfill the
requirements of the contract. Notwithstanding any provisions herein, in the event sufficient funds for
the performance of this Agreement are not appropriated by TPCG in any fiscal year covered by this
contract, this agreement may be terminated by the TPCG giving notice to the Contractor of such facts
and the TPCG's intention to terminate its financial obligation.
TERMINATION OF AGREEMENT FOR CAUSE:
The TPCG may terminate the contract for cause based upon the failure of the Contractor to comply
with the terms and/or conditions of the contract, or failure to fulfill its performance obligations
pursuant to the contract, provided that the TPCG shall give the Contractor written notice specifying the
Contractor's failure. If within thirty (30) days after receipt of such notice, the Contractor has not
corrected the failure or, in the case of failure which cannot be corrected in thirty (30) days, begun in
good faith to correct such failure, and thereafter proceeded diligently to complete such correction,
then the TPCG may, at its option, place the Contractor in default and the contract shall terminate on
the date specified in such notice.
The Contractor may exercise any rights available to it under Louisiana law to terminate for cause upon
the failure of the TPCG to comply with the terms and conditions of the contract, provided that the
Contractor shall give the TPCG written notice specifying the TPCG's failure and a reasonable
opportunity for the TPCG to cure the defect.
TERMINATION OF AGREEMENT FOR CONVENIENCE:
The TPCG may terminate the contract at any time by giving 30 days written notice to the Contractor
of such termination or negotiating with the Contractor an effective date. The Contractor shall be
entitled to payment for deliverables in progress, to the extent work has been performed satisfactorily.
DAVIS BACON-ACT:
All prime construction contracts in excess of $2,000 awarded by no-federal entities must include a
provision for compliance with the Davis-Bacon Act (40 U.S.C. 3141-3144 and 3146-3148) as
supplemented by the Department of Labor regulations and 29 C.F.R. Part 5 (Labor Standards Provisions
Applicable to Contracts Covering Federally Financed and Assisted Construction). See 2 C.F.R. Part 200,
Appendix II(D). In accordance with the statue, contractors must be required to pay wages to labors and
mechanics at a rate not less than the prevailing wages specified in a wage determination made by the
Secretary of Labor. In addition, contractors must be required to pay wages not less than once a week.
ATTESTATIONS AFFIDAVIT: the fully executed affidavit must be submitted with this quotation, which
is included as part of this solicitation package.
NON-COLLUSION AFFIDAVIT: the fully executed affidavit must be submitted with this quotation,
which is included as part of this solicitation package.
CONTINUING OBLIGATION: Contractor has a continuing obligation to disclose any suspensions or
debarment by any government entity, including but not limited to General Services Administration
(GSA). Failure to disclose may constitute grounds for suspension and/or termination of the Contract
and debarment from future contracts.

QUOTATION FORMS
Quotes shall be made on the blank forms furnished (Official Quotation Form Section "A") with this
packet. Amounts shall be in both words and figures, and in case of discrepancy, words shall govern.
All signatures shall be in long hand, executed by a principal or officer authorized to make contracts.
The Contractor's legal name, email, physical address, and phone number shall be fully stated.
VENDOR REGISTRATION:
The Terrebonne Parish Consolidated Government Purchasing Division requires vendors to register
online at https://secure.tpcg.org/vendor/ .This tool is part of our efforts to make it easier for you to do
business with the Parish, as well as provide you with better business opportunities.
If you have already taken action to complete this requirement, you do not have to complete this
process again. However, if you have not already registered online as a vendor you will need to do so
within ten (10) days' notice of award of this quotation.
TAXES:
It is understood that all applicable taxes are included in the quoted price. Successful Contractor as an
authorized agent of OWNER will be authorized by OWNER to receive an exemption from Louisiana
State Sales taxes related to the work to be performed. As such, the quotation price should reflect this
exemption.
PAYMENT:
Payments are predicated upon successful completion and written approval by TPCG Administration or
designee of the described tasks and deliverables as provided in the Scope of Services (as applicable).
Payments will be made to the successful Contractor after the TPCG approves in writing the work
performed and the submitted invoice(s). Payment will be made only after TPCG approves the invoice(s)
for payment. The TPCG will make every reasonable effort to make payments within thirty (30) calendar
days of an approved invoice that falls under a valid contract.

Specifications
The Contractor shall furnish all labor, materials, equipment, tools, transportation and supplies required
to install movable; modular walls that support power, data and security cabling, dropped grid acoustical
ceiling tiles and planters. Shipping costs must be included in unit pricing.
GENERAL: Whenever materials or equipment are specified or described in the documents by using the
name of a certain brand, make, supplier, manufacturer, or definite specification; the naming or
specification of the item is only intended to denote the quality standard of the item desired and to
convey and establish the general style, type, character and quality of material , equipment or product
desired and does not restrict respondents to the specific brand, make, manufacturer, or specification
named; and that equivalent products may be acceptable.
The respondent must specify the brand/stock/model numbers of the product offered in his/her quote.
Quotations not specifying brand/model/stock numbers shall be considered as offering the exact
products specified in the solicitation.
Telescopic Glass Wall (Scope of Work):
* 20' Telescoping glass wall with (2) stationary glass walls and (1) stationary solid wall.
* Center position will open and telescope left/right
* Telescoping glass wall must have semi-opaque film applied through the center portion of the glass.
* Includes removal of the current accordion style wall and all tracking hardware prior to installation of
the glass wall.
Basis of Design manufacturers shall include DIRTT, Modernfold, Hufcor, NanaWall, Muraflex, or approved
