The project consists of the furnishing of all labor, materials and equipment, as further defined herein,
for the removal and installation of sidewalk located within the City of Collegedale, as indicated on the
plan sheets attached to these specifications.
The Owner reserves the right to wave any informalities or to reject any, any part or all of the bids, and
to extend the date for the receipt of bids.
Bidders must comply with all Local, State and Federal requirements for bidding, construction or
installation of the work. Bidders must comply with all conditions provided in the Information for

I. BACKGROUND
The City of Collegedale is seeking bids for the removal and installation of approximately 2,350 square
feet of a five (5) foot wide concrete sidewalk at the location further identified in these contract
specifications. This is an estimated amount, and final quantities may differ. The City reserves the right to
increase or decrease the quantities specified.
II. SUMMARY OF WORK
The project covers the site preparation, forming and installation of a concrete sidewalk as shown on the
plans attached to these specifications. Work includes, but is not limited to, the following:
1. Removal of existing concrete sidewalk
2. Site preparation/excavation to accommodate new 10” concrete sidewalk section
3. Installation of 5” thick concrete sidewalk per City standard detail
Bidders must comply with all Local, State and Federal requirements for bidding, construction or
installation of the work.
III. GENERAL REQUIREMENTS
A. Vendor Requirements
1. The work consists of non-safety related commercial scopes and shall be in accordance with the
Vendor’s quality assurance program or procedures.
2. An on-site kick-off meeting is required. Additional on-site meetings will be scheduled as needed.
3. Project team personnel that will be directly involved with the project, including subcontract
companies and subcontract company personnel, are to be identified to the Owner for approval.
No substitutions of key personnel, subcontract companies, or subcontractor key personnel, are to
be made after the bid submittal and award of contract without Owner’s specific written approval.
4. The work to be performed under this Contract shall be proposed as a unit price or lump sum, as
provided, which shall include all work necessary to complete the project as shown, specified, and
directed, and delivered to the Owner and subject to the approval of the Owner, the Engineer, and
the applicable Officials having jurisdiction over the project. The items shown in the proposal shall
be complete and operational for the prices provided in the bid. The bid shall anticipate work being
done in contract time allotted.
5. Perform acceptance testing.
6. Work hours are to be 7 a.m. to 5 p.m. Monday through Thursday.
7. Prepare and maintain a budget summary report showing detailed and itemized monthly accruals,
milestone (or monthly) billing, payments to date, remaining spend, invoice numbers, payment
status, etc. To be prepared and submitted monthly. Submitted invoices should correspond to the
budget summary report in level of detail, etc.
8. Obtain prior Owner approval in advance of changes in key project personnel (including replacing,
backfilling, adding additional, etc.), including subcontract personnel.
9. Any work performed in a confined space must be done in accordance with TOSHA requirements
and federal law. Collegedale must be notified before any confined space entry is performed.
10. Ensure key project personnel (technical, project management, technical writers, etc.), including
subcontractor personnel, are routine participants in project status and coordination meetings.
