DEPARTMENT OF REHABILITATION AND CORRECTIONS, 2900 CARNEGIE AVENUE, CLEVELAND , OHIO 44115 REQUEST FOR DESIGN SERVICES, PRODUCT , AND INSTALLATION QUOTE

On behalf of the Department of Rehabilitation and Corrections (DRC), please provide a quote for a front and rear entry door contact and 24/7 monitoring.
on the 1st floor of 2900 Carnegie Ave., Cleveland, Ohio 44115. The work will be done during normal business hours. All services are to be completed within two weeks of award.

GENERAL INFORMATION
• Vendor will obtain and abide by all rules and regulations from the state entity and the building property manager
• Vendor will be responsible for keeping the site orderly, clean , and safe at all times.

DELIVERABLES:
1. Quote should be detailed to include each product with description and product number
2. Quote should indicate with separate columns the item List Price, Discount Percent, and Purchase Price

BUILDING ACCESS
2900 Carnegie Avenue
Located off Route 90, next to the KFC Chicken restaurant. There is a rear and front single door for building access. One-level building and no stairs.

MANDATORY SITE VISIT:
