TO ALL PROSPECTIVE BIDDERS:

The following questions were submitted by a potential vendor. Our answer is listed

in red below:

1. How many delivery locations for the fentanyl test strips? Do you have

the address/addresses of the locations? 814 Jefferson Ave, Memphis

TN only.

2. If we have to send to multiple delivery locations, will each of them be

ordering directly, or all through the same channel? Wha’s the MOQ per

location, per order? There is only 1 delivery location. Yes, all through

the same channel.

3. Is 8,500 boxes an estimate only or the actual order for the bid? If it is

just an estimate, is there any range for the actual quantity? Yes, 8,500

is an estimate. No range at this time.

4. We are offering Fentanyl test strips in 10 and 100 strips/box. Each strip

is individually packaged with QR code for instructions. Although our 10-

pack is available, it will have a longer lead time for delivery and higher

shipping costs than the 100-pack. Will you be open to consider both 10

ad 100-pack on our bid? Only 10 per box.

5. Would you be open to consider different F.O.B pricing for our two

different pack sizes, considering shipping cost/test strip is a major

factor for pricing. Not sure what you are asking, do you mean

considering FOB pricing for box sizes for Naloxone and FTS?

6. Can the Drug-Free Workplace Affidavit be notarized by a public

notary in Canada? (As a note, the person designated to work o this

bid resides in Canada) Yes.
