TO ALL PROSPECTIVE BIDDERS:

The following questions were submitted by a potential vendor. Our answer is listed

in red below:

1. Will Shelby County accept bids on Naloxone HCI Nasal Spray 8 mg (2

per box)? Naloxone HCL should be 4 mg per box.

2. I see in the bid that a sample of the product may be requested? Will this

be a necessary step for us to provide since we already provide this

medication to the county? Yes.

3. As we are a licensed pharmacy we do not have samples, and it is illegal

to provide. I can send a picture of the box? Samples must be provided.

4. ________________ is a leader in distributing rapid diagnostic tests with

over 250 million strips sold annually. Many companies try to replicate

the business of __________, even its name. ___________ stands

apart and ahead of her competitors with high quality products and high-

end personal customer service. ____________ is interested in

and Fentanyl Test Strips. In this regard last year’s price of above said

bid is needed for quote the best price. In this regard, last year’s price is
