TO ALL PROSPECTIVE BIDDERS:

The following questions were submitted by a potential bidder. Our answers

are listed below in red:

1. Completion of the LOSB Utilization Plan Form (20%)? Completion of this form is

a requirement.

2. EOC Compliance Form B? Form B is to be completed/signed, with applicable

backup documentation if the required goal is not met on Form A.

3. Since we are a small business and minority owned in Shelby County will these 2

forms need to be complete by us? These forms must be completed by the

company submitting the bid, listing the selected LOSB subcontractors.

4. On Wednesday, November 12, 2025, at Pre-Bid meeting will the interested

bidders be taken to each site. Or are we allowed access to these locations

before the bid deadline? As soon as the meeting is over, all interested parties will

be escorted to the locations that are located inside of fencing.

5. Are the contracts for these locations 12 month? If so, please let me know the

start date and end date. This contract should be in place for the full 12 months.

The start and end dates are TBD.

6. Is there a performance bond or any other bond requirement for bidding? This will

be discussed during the meeting.

7. Shelby County Purchase sheet with the locations listed on it or can we create a

similar form with the locations listed with prices per month and attached with the

bid? Please use the form provided.
