Good morning and thank you for showing interest in our solicitation!
As specified in Roll-Off Services 27-0051 ITB page 18 under Bid Submittals it lists the Certification Page (Attachment A), SMBE (Attachment B), EHD (Attachment C), and Commercial Reference Form (Attachment D), which are wrapped up together into RollOffServ270051 BID RESPONSE.pdf, as being required to be submitted with the bid. On the same page under Prior to Award or May Submit with Bid it lists the Affidavit Regarding The Use of Coercion for Labor and Services (Attachment E) and the Affidavit Regarding the Use of Companies Owned or Controlled by a Foreign Country of Concern (Attachment F), which are wrapped up in RollOffServ270051 BID RESPONSE 2.pdf, as NOT having to be submitted with the bid but they do need to be submitted prior to award.
