PRESCRIPTION SAFETY GLASSES
I. STATEMENT OF WORK
A. SCOPE
The scope of this RFQ is to provide District employees with prescription safety
eyeglasses, as needed and as ordered by the District.
East Bay Municipal Utility District (District) intends to award a three-year contract (with
2 options to renew for one-year terms) to the Bidder(s) who best meets the District’s
requirements.
B. MINIMUM QUALIFICATIONS
1. Minimum Qualifications
a. Bidder’s principal or staff shall have been regularly engaged in the
business of providing prescription safety glasses for at least five (5) years.
b. Bidder shall possess all permits, licenses, and professional credentials
necessary to perform services as specified under this RFQ.
C. SPECIFIC REQUIREMENTS
1. REQUIREMENTS - Under these specifications, it is required that prescription
safety glasses be provided to District employees delivered f.o.b. to EBMUD
Receiving Dock, 1010 Franklin Street, MS 102, Oakland, California 94607, Attn.:
Rosana Polanco, as ordered.
2. The vendor shall provide:
a. In-house order-taking, delivery, repair, and fitting of prescription safety
eyeglasses provided at an hourly rate in Oakland. CA.
b. Safety glasses will meet ANSI Z87.1 standards.
c. EBMUD requires in-house order taking.
d. Eyeglasses are to be furnished with cases.
e. Provide one-year warranty for frames and lenses due to defects or doctor
prescription error. Changes in prescription are to be covered (redone) at
no charge within 60 days.

D. DELIVERABLES
The Contractor must provide safety frames and lenses that meet or exceed ANSI Z87.1
standards and have sufficient resources to fulfill orders within 4 weeks. Contractor will
meet with District staff on-site in Oakland twice per month, for two hours per site visit,
and fit personnel with safety frames and lenses with employee provided prescription.
Contractor will also assess problems and remedy fit and prescription deficiencies during
these site visits.
Deliveries will be accepted between the hours of 8:00 a.m. and 4:30 p.m., Monday
through Friday. No deliveries will be accepted on Saturdays, Sundays or District holidays.
The District will inspect product after its arrival at the delivery point. Contractor is solely
responsible for ensuring the product arrives at the District’s ship-to location free of
defects and manufactured in strict conformance with the specifications.
E. OTHER REQUIREMENTS
1. The Contractor’s personnel and equipment will be allowed reasonable access to
the work site during normal business hours, and a sufficient area of working
space. Unless otherwise agreed by both parties, normal business hours shall
include an eight-hour workday during the interval of 0700 and 1700 hours on
Monday through Friday and exclude work on weekends or District holidays.
District holidays are the following:

Thanksgiving Day and day after
Christmas Day and day after
Contractor’s personnel are required to participate in District safety training. Time shall
be included in the Contractor’s bid.

2. SITE SECURITY
The Contractor shall comply with the District’s protocol for personnel identification,
site access control, and contractor deliveries.
The Contractor shall designate a site security monitor who shall be on-site and
available at all times while work is being performed, and who shall ensure that
requirements of this section are met. This individual may be the superintendent.
For each day of on-site work, all Contractor, subcontractor and delivery personnel
shall sign a daily sign-in log furnished by the District site manager. Each daily log
shall include: individuals’ full name, company and company phone number.
Personnel Identification and Badging. Contractor’s personnel and all people
associated with the work will be issued individual District photo identification
badges that will be valid for two years. After two years, the contractor badge
needs to be renewed. Contactor shall submit a written request to the District for
an extension of the validity of the badges.
The District will provide project specific identification badges for use by
Contractor’s staff and all staff associated with the work. There will be two types of
identification badges required: (1) photographic identification badges for people
who will work at the site more than 5 continuous days; and (2) generic temporary
badges for those people who will only work at the site on an infrequent basis will
be escorted by District staff.
Contractor shall make arrangements for badging with the District Contact listed in
Section IV.A. of this RFP to schedule a day for project personnel, including those of
dive subcontractor, to report to either the Main Administration Building located at
375 Eleventh Street, Oakland, second floor, or the Wastewater Treatment Plant or
Pardee, depending on where they will be working. The District sponsor will work
with Security to schedule contractor badging by submitting the K-073A Eform.
Virtual badging may be available by having the District sponsor email a copy of the
contractor’s driver’s license and a headshot (showing head and shoulders) with a
blue background (no sunglasses, hats, or safety helmets). Send the driver’s license
and headshot to secadmin@ebmud.com for processing with the K-073A form.
Badging hours are typically Tuesdays from 10:00am-11:00 am, and Wednesdays
from 1:30 pm-2:30 pm at the EBMUD Administration Building at 375 11th Street in
Oakland, on the second floor. The contractor shall work with the District sponsor
to get badging information. The District sponsor will work with Security to provide
answers to their contractor. If the contractor gets badged at the other locations,
the District sponsor will schedule an appointment with those locations to get
information on badging protocols.
