REQUEST FOR QUALIFICATIONS
ON CALL MATERIAL TESTING SERVICES

This Request for Qualifications (RFQ) seeks qualified firms to provide on-call material testing
services for the City of Sequim. An on-call contract for consulting services will be awarded to
selected respondents in an amount not to exceed $250,000.00. The contract term will be for 3
years and may be extended at the discretion of the City to the maximum 4 years allowed by
RCW 35.23.352(13)(c).
The City reserves the right to amend the terms of this RFQ, to circulate various addenda, or to
withdraw the RFQ at any time, regardless of how much time and effort consultants have spent
on their responses.
Description of Services
Services shall include the sampling and testing of roadway and building construction materials
including, but not limited to: hot mix asphalt, cement concrete, grout, aggregates (base course,
top course, pipe zone bedding, etc.), and soils. Firms selected will receive an On-Call task order
agreement. Individual tasks will be negotiated as stand-alone assignments terminating when
the task budget is exhausted, or the task time frame has expired. Tasks will be related to
projects listed on the City’s Capital Improvement Program (CIP) and/or related to commercial
and residential development where needed. The selected firm(s) shall be familiar with
applicable American Association of State Highway and Transportation Officials (AASHTO),
Washington State Department of Transportation (WSDOT), and American Concrete Institute
(ACI) standards and testing methods.

Submittal of Statement of Qualifications
Statements of Qualifications in response to this RFQ must include the following information:
• Consultant or firm name, phone numbers, mail and email addresses
• Resumés of key personnel (Up to three (3) single-sided 8.5”x11” Page Max)
• List of past projects with brief description and reference contact information for each
(Up to three (3) single-sided 8.5”x11” Page Max
• A one (1) page cover letter (optional)
Please submit an electronic copy of your Statement of Qualifications in pdf format to: Public
Works, at pubworks@sequimwa.gov no later than the Response Deadline above. Submittals
will not be accepted after that time and date. Any questions regarding this RFQ should be

Evaluation Criteria
Submittals will be evaluated and ranked based on the following criteria:
1) Qualifications (Accreditations/Expertise/Capacity of Firm);

2) Key personnel, including point of contact for task orders and any subconsultants
(Including Testing Certifications);
3) Relevant project experience, specifically on-call contract experience with municipalities
or other government entities.
