    Medical, Dental, Vision, Prescription Insurance and EAP Benefits Coverage
    February 12, 2026
    This Addendum is issued in accordance with the RFP Terms & Conditions and is intended to provide
    additional information and clarification to Respondents.
    This Addendum consists of one (1) item which is listed below and is an integral part of the RFP to which
    it is attached and is incorporated therein and made part thereof by this reference.
    Item 1 – Questions and Answers
    1.1 Are there specific documents pertaining to the EAP portion of the RFP? There is no mention of the
    requirements for the EAP other than the “Wellness Objectives” which is not EAP.
    Answer: Documentation related to the EAP portion of the RFP will be provided by OneDigital to
    all interested parties in separate, secure communication.
    1.2 Please send any reporting information and data pertaining to the RFP.
    Answer: All available reporting information and data pertaining to the RFP will be provided to all
    interested parties by OneDigital via separate, secure communication.
    1.3 On the 10/1/2025 ASO renewal file, we are only seeing a composite rate for their Specific Stop
    Loss premium. Assuming that this group not have aggregate coverage? We do see the fully funded
    equivalents and are using those for comparison purposes, but we still would like to review the
    final/signed stop loss policy (if available) to check for any lasers as well.
    Answer: Spotsylvania County does not carry aggregate stop-loss coverage. Signed ASA and stop-
    loss agreements are not available. Unsigned copies are included in the addendum data and will be
    provided to all interested parties by OneDigital via separate, secure communication.
    1.4 Additional detail related to identifying Rx utilization for large claimants – including any available
    reporting for the #2 large claimant with “Mx Myeloma & Malig Plasma Cell Neo.”
    Answer: The County can confirm that the member is currently in active and on-going treatment,
    however, detailed case notes or additional reporting beyond what is included in the addendum data
    are not available.

    Answer: Large claimant reporting for the period January 1, 2024 through December 31, 2024 is
    included in Attachment B to this Addendum.
    1.6 Any broker commissions for OneDigital that we should include in our proposal?
    Answer: The County issues the following clarification regarding broker compensation and proposal
    pricing requirements:
    Section III – Statement of Needs states that cost proposals shall be quoted net of commissions.
    To ensure clarity and uniform proposal pricing, the below clarification modifies and supersedes any
    conflicting language in Section III – Statement of Needs regarding commission.
    The County provides the following direction:
    (a) Medical Coverage
    Offerors proposing medical coverage shall include broker compensation in the amount of $6.50
    per employee per month (PEPM) payable to OneDigital. Medical rates submitted shall
    include this commission amount.
    (b) Dental and Vision Coverage
    Offerors proposing dental and/or vision coverage shall submit rates net of commissions. No
    broker commission shall be embedded in dental or vision rates.
    (c) Evaluation of Pricing
    For purposes of evaluation, the County will consider total program cost inclusive of broker
    compensation where applicable. Offerors shall clearly identify whether broker compensation is
    included in the submitted rates.
    1.7 The TPA OneDigital would like JHP to assume (with TPA Admin fee, if possible).
    Answer: The County will not designate or select a TPA for quoting. Offerors shall propose the
    TPA solution that best accomplishes the goals of the RFP and best serves the employees of
    Spotsylvania County.
    1.8 In the RFP, Section V requests a “Completed Wellness Exhibit”. Is this a separate document? If so,
    please provide this required document. However, if it is not a separate document, we will be
    including responses to “Sections F Wellness” and “D. Clinical Quality Indicators” in this section.
    Answer: Please disregard the request for the Completed Wellness Exhibit. No such document is
    included in the RFP. Offerors shall instead describe any wellness programs their firm offers that
    would be beneficial to the County.
    1.9 We do not see an actual Rx claim file that could be used for a claim/claim analysis to demonstrate
    the savings potential from our solutions.
    Answer: A complete Rx claims file is not available. Available information is limited to the top 25
    drugs and diagnosis.
    1.10 Please provide a census that includes all eligible waivers.
    Answer: A complete eligible census, including waiver information, will be provided to all
    interested parties by OneDigital via separate, secure communication.
    1.11 Please provide Dental and Vision rates for the current plan year as well as a 5-year history
    Answer: Dental and Vision rates are currently combined with the medical rates for the applicable
    tier of coverage. Separate standalone Dental and Vision rate histories are not available.
    1.12 Please confirm the broker commission amount included in the Dental and Vision plans.
    Answer: Offerors shall quote Dental & Vision net of broker commission.
    1.13 The Geo Access parameters in the RFP document refer to the Medical plan. Please confirm the

    parameters needed for Dental and Vision.
    Answer: Offerors shall apply the same Geo Access parameters used for the Medical plan to the
    Dental and Vision plans.
    1.14 Please confirm if the Vision Plan should be considered Voluntary (no employer contribution to the
    rates) or Contributory.
    Answer: The Vision plan is currently bundled with the medical plan. Offerors shall propose rates
    and benefits plan designs for a standalone Vision plan, not bundled with Medical.
    1.15 In order to provide a provider disruption report we must receive the current in use providers from
    the incumbent carrier in excel. This file must contain provider TIN, provider first name, provider
    last name, provider address, city state and zip.
    Answer: Provider disruption data will be included as part of the addendum data set and will be
    provided to all interested parties by OneDigital via separate, secure communication.
    1.16 For respondents not proposing medical/pharmacy benefits, should those sections be removed from
    the table of contents and renumbered, or retained as placeholders for tab consistency with a “Not
    Applicable” notation?
    Answer: Offerors not proposing Medical and/or Pharmacy benefits shall retain the applicable
    sections for tab consistency and indicate “not applicable” in their responses.
    1.17 A. Please confirm whether “rates shall remain firm for the initial five years” is intended to require
    flat rates for the full five-year term, or whether a disclosed multi-year rate guarantee structure (e.g.,
    defined rate guarantees with stated renewal caps) is acceptable.
    B. Additionally, please confirm whether the referenced Medical CPI is intended to apply to dental
    and vision benefits, and that any post–year five rate adjustment is capped at the lesser of CPI or 3%
    on a per-renewal year basis (non-cumulative), and not subject to adjustment due to standard
    underwriting factors (e.g., enrollment changes).
    Answer: The purpose of this requirement is to allow the County to evaluate and plan for a multi-
    year rate structure. Offerors are not required to propose flat rates for the full five-year term;
    however, flat or stable rate structures are preferred. Multi-year rate guarantees with clearly
    disclosed renewal terms and caps are acceptable.
    The reference to Medical CPI is intended to support long-term planning. Offerors should clearly
    disclose how any proposed rate adjustments would apply to Medical, Dental, and Vision benefits,
    including any post–year five adjustments, caps, or assumptions.
    1.18 A. The census includes retirees but does not identify which individuals are post-65. Based on our
    review of dates of birth, we estimate there are approximately 43 post-65 retirees enrolled in the
    dental plan. Please confirm whether this estimate is accurate so we can rate the plan both including
    and excluding post-65 retirees, as requested.
    Answer: The census provided includes date of birth and plan enrollment information. The plan
    currently allows post-65 retirees to remain enrolled. Offerors shall use the provided census as the
    basis for their proposed rates.
    B. If the above estimate is not accurate, please clarify the correct number of post-65 retirees
    enrolled in dental.
    Answer: The County is not providing a separate count of post-65 dental retirees. The census
    includes date of birth and plan enrollment information, and offerors shall use the provided census as
    the basis for their proposed rates.
    1.19 Additionally, the PDF titled “SPOTSYLVANIA COUNTY (All Retirees over 65)_retiree over
    65_20251208 12_54 PM” contains medical and pharmacy claims for post-65 retirees, but we did

    not see corresponding dental or vision claims data. If available, please provide dental and vision
    claims for post-65 retirees. This information is particularly relevant if the post-65 dental population
    differs from the estimated 43 lives.
    Answer: Dental and vision data claims data cannot be segmented by post-65 retirees. Accordingly,
    separate Dental or Vision claims reporting for post-65 retirees is not available.
    1.20 Is it possible to get a Rx Claims file:
    Answer: A complete prescription claims file is not available. Available information is limited to
    the top 25 drugs included in the original data submission Data set will be provided again to all
    interested parties by OneDigital via a separate, secure communication.
    1.21 Please provide the Dental/Vision Rates Current & Renewal
    Answer: Current Dental and Vision rate information will be provided to all interested parties by
    OneDigital via separate, secure communication. Please note that the client's medical, dental, and
    vision premiums are currently combined into a single rate. In addition, a separate Vision
    administrative fee of $1.13 per month applies. Dental carrier rates are listed below:

    1.22 Would you be able to send me the top provider/facility list?
    Answer: A network analysis including the top provider and facility information will be provided to
    all interested parties by OneDigital via separate, secure communication.
    1.23 Is the current ASO Dental integral to Medical? Is the Medical Plan Grandfathered as defined by
    ACA? If the dental ASO plan is integral to medical, this is not something our systems can
    administer and we would not be able to quote on this group
    Answer: The Dental ASO rate is separate from medical plan; Premiums for medical, dental, and
    vision are paid together by employees, while the Dental ASO arrangement is separate from
    Medical.
    1.24 Can you please provide a copy of the census showing all eligible employees as well as their home
    zip code?
    Answer: A census including all eligible employees and home ZIP code information is included in
    the addendum data and will be provided to all interested parties by OneDigital via separate, secure
    communication.
    1.25 Can you please provide a copy of the full dental booklets with procedures and frequencies?
    Answer: The dental summary of benefits will be provided to interested parties by OneDigital in a
    separate, secure email.
    1.26 What are the current and renewal rates for both dental and vision?
    Answer: The dental summary of benefits (three-page document) is included in the addendum
    materials and will be provided to all interested parties by OneDigital via separate, secure
    communication. Full dental booklets are not available.
    1.27 Are you able to get all providers used within the last 12 months along with the total submitted
    charges and paid claims for each provider? In addition to this Provider Disruption information, we
    like to see if the current carrier can provide us with Financial Disruption information. In this report,

    we would look for the following data elements in an excel worksheet:
    Provider Tax ID Number
    Provider Name
    Provider Address
    Provider City

    Provider Network Indicator for Current Carrier

    Answer: Available data will be provided to all interested parties by OneDigital via separate, secure
    communication.
    1.28 A. Is the current dental plan ASO or fully insured?
    B. If fully insured, can you please provide current rates?
    C. If ASO, can you please provide the current ASO fee and any shared savings?
    Answer:
    A. The current dental plan is administered on an ASO basis.
    B. Not applicable.
    C. The current ASO fees and enrollment information from the most recent renewal are included in
    the addendum materials and will be provided to all interested parties by OneDigital via separate,
    secure communication.
    1.29 May you confirm the effective date you are asking for on this dental proposal?
    Answer: Dental services shall be effective as of October 1, 2026.
    1.30 May you supply a provider file to us so we can complete a disruption analysis?
    Answer: Provider disruption data will be provided to all interested parties by OneDigital in a
    separate, secure communication.
    1.31 May you confirm that the dental trend exceeds the 3% rate of inflation? Would you be able to agree
    to the following:
    Fully insured offering of two-year rate guarantee and a third-year cap exceeding 5%
    Answer: The trend is based on market volatility. If there are rate guarantees, Offerors shall clearly
    disclose those guarantees in the financial documents.
    1.32 Does the dental insurance today cover active employees, pre-65 retirees, and post 65Retirees?
    Answer: Yes, dental vision covers active employees, pre- and post-65 retirees who elect to enroll.
    1.33 Do you want dental proposals with and without the post 65 retirees covered?
    Answer: Offerors are not required to submit separate proposals excluding post-65 retirees.
    However, if the removal of post-65 retirees would result in a change to proposed rates, that
    potential impact must be clearly disclosed and reported in the proposal.
    1.34 Can you please provide dental claims without the post 65 retirees covered?
    Answer: Dental claims data cannot be segmented to exclude post-65 retirees. Accordingly, separate
    dental claims reporting without post-65 retirees is not available.
