RFP #RFP 23-19 - The Long COVID Awareness Project
The County of San Joaquin is soliciting Request for Proposals (RFP) for its Public Health Services(PHS) Department. San Joaquin County Public Health Services’ (SJCPHS) Emerging Diseases Program is seeking proposals to improve the health outcome of patients who live in medically underserved communities in San Joaquin County and have experienced Long COVID and other complex chronic conditions with similar symptoms post- COVID -19 infection. SJCPHP wants to accomplish this through public awareness and availability of resources as described below. The successful proposal shall include a timeline of activities and detail how the Contractor will address the following items: Objective 1: Public Information Campaign Content Development Provide accurate information of Long COVID based on current knowledge and clinical advancements and work collaboratively with SJCPHS staff or designee to develop messaging and collateral for public health campaigns. The information will be used to educate patients in a culturally and linguistically appropriate manner, urging individuals experiencing Long COVID symptoms to seek medical care with their primary care providers. Objective 2: Web-based Long COVID Assessment Tool Develop a web-based self-guided triage and assessment tool in a linguistically and culturally appropriate manner. This project must provide health education detailing behavioral and lifestyle modification recommendations that could help improve the symptoms of Long COVID through self-care practices. Objective 3: Clinicians Education Tool Kit Develop a set of educational modules for healthcare providers to learn about clinical testing for patients who present with Long COVID or other similar complex symptoms post- COVID-19 infection along with updated pharmacological and behavioral treatments applicable to this condition. Objective 4: Presentations for the San Joaquin Medical Community Develop and deliver presentations of technical assistance for the San Joaquin medical community. The information may provide updated information on Long COVID symptoms, diagnostic testing options, pharmacological and behavioral treatments, and options providers may use to assist their patients’ efforts in acquiring disability accommodations. Additionally, an evaluation survey will need to be developed for presentation participants to complete; SJCPHS will require summarized result reporting and the completed originals.
