In accordance with the provisions of the Municipal Code of the City of Redding, proposals must be
submitted to the Purchasing Division Office, 3rd floor, located at City Hall, 777 Cypress Avenue, Redding,
California 96001 prior to 2:00 p.m. PT, Thursday, June 4, 2026, for furnishing the City of Redding a
proposal for providing an industrial injury treatment program per the attached specifications and general
conditions.
PROPOSALS RECEIVED AFTER THIS TIME AND DATE MAY NOT BE ACCEPTED OR

The said proposals will be opened at 2:00 p.m. PT, on Thursday, June 4, 2026, in the Purchasing

INTRODUCTION
The City of Redding invites qualified medical Providers (also referred to as Provider(s) or
Proposer(s) in this RFP) to submit proposals to provide treatment of industrially injured workers.
The award of resulting contract shall be based on the ability to perform and provide valued service
and quality treatment in a timely manner is the ultimate objective.
Annually, it is estimated that there will be 375 industrial injury exams/evaluations. These numbers
are estimates only.
MAJOR RESPONSIBILITIES
The successful medical Provider(s) will be expected to:
1. Ensure Industrial injury patients are shown preference over patients with routine
appointments in an effort to minimize their waiting time since they are on duty; as well
as ensure that applicants/employees wait no longer than one hour, from the time of
actual appointment, before services are begun. This should not be interpreted to mean
that an Applicant/Employee could wait in the waiting room for 55 minutes and then be
placed in an examination room to wait for another 60 minutes. If an Employee waits
more than one hour, from the time of actual appointment, before services are begun, the
fees will be discounted 50%.
2. Provide a medical director who will be responsible for providing quality licensed
examining physicians and all appropriate support staff, such as licensed nurses, medical
technicians, work comp coordinator etc. who are experienced in occupational medicine and
familiar with State and Federal laws. Provider(s) must indicate in their proposal if
physicians and/or support staff are located at a different location.
a. Provide a specific contact list for the City Representative, e.g., contact for
information on appointments, treatment provided, medical questions, treatment
protocols, billing information, etc.
3. Provide all necessary facilities, equipment, supplies and materials to carry out the contract.
4. All medical reports for industrial injury patients must delineate treatment, specific body
part and medications. All other non-industrial medical issues, medications and/or
treatment shall be noted in the history section of the medical report.
a. All medical reports shall be in compliance with State laws and regulations.
5. Provider shall ensure industrial injury patients prescribed opioids sign a contract and
instructed that medication will be accounted for and blood tests may be ordered to confirm
appropriate levels.
6. Work with City to fully evaluate and respond to any appeals and provide clarification,
expert testimony, or assistance as necessary and as requested.
7. Maintain and store all medical records for the term of the contract in a secure and
confidential manner, at no additional cost
8. Keep all medical records indefinitely and make them available only to the City Risk
Manager, or his/her designee. Should the Contract be cancelled or expire, these records
are the property of the City of Redding and shall be made available for transfer to the
appropriate medical provider as assigned by the City within two (2) weeks of
cancellation or expiration, at no charge to the City.
9. Provide monthly billing to the City's bill review vendor no later than the 15th of the month
following the month in which services were provided. Invoices are to be provided on forms
and in the format as outlined in the California Labor Code.
10. Identify and report to the City Representative any conflicts or problems that arise between
the employee and the medical Provider(s).
11. Keep the City informed of recent changes in laws and judicial decisions pertinent to
industrial treatment guidelines.
TERM OF CONTRACT
The contract term shall be July 1, 2026 through June 30, 2029. Contract may be renewed for a
maximum of two (2) consecutive one year periods, subject to satisfactory performance, and
agreement to continue to operate under the same pricing, terms, and conditions, unless mutually
agreed otherwise.
PROCEDURAL INSTRUCTIONS FOR SUBMITTING A PROPOSAL
Proposals must be responsive to this Request for Proposals as issued. Proposals made subject to
conditions or limitations specified by the respondent may be rejected. By submitting a proposal
for the work, the Provider agrees to utilize the City's standard form for Consulting and Professional
Services Agreement (Exhibit A). Please see the required levels of insurance coverage set forth
in the professional services agreement (Section 5).
The proposal shall include, as a minimum, the following information:
Cover/Transmittal Letter - Letter must be signed by a member of the organization who has
the authority to enter into contracts on behalf of the organization. The letter must
acknowledge receipt of any and all addenda (by number), if any have been issued.
Identify and include a brief resume of the principal of your firm who will be primarily
responsible for services provided to the City. The City anticipates that the firm will take
great care in the designation of a lead individual. It is also anticipated that the lead
individual will be dedicated to providing quality and timely services for the City at times
deemed necessary by the City.
A description of the firm's experience in performing this type of work, process and
procedures, including, at minimum, three (3) references that have had contracts with you
of similar size and scope in which the City may contact (name, telephone number,
location).
A declaration of the firm's ability and willingness to commit and maintain staffing, both
number and level, to successfully provide the services during normal working hours.
A discussion of the primary factors you believe the City should consider in selecting your
firm.
Include all applicable certifications and or documentation regarding your firm's
knowledge of local, State and Federal regulations regarding safety compliance, employee
health and safety, specifically, Cal-OSHA. This information may be submitted by way of
USB drive.
Each proposal package shall contain the original (unbound) proposal and an exact copy of the
proposal in PDF format on a USB Drive. Both the package containing the proposals as well as the
individual envelope are to be marked with the Provider's name, "RFP Schedule No. 5702,
opening at 2:00 p.m. PT on Thursday, June 4, 2026" and submitted to:

The proposals will be opened at 2:00 p.m. in the Purchasing Division. 3rd floor of City Hall. All
proposals received after the scheduled closing time for receipt of proposals will not be accepted or
considered, and will be returned to the respondent unopened.

EVALUATION FACTORS
Responsive proposals received in reply to this Request for Proposals will be evaluated based on
the following criteria:
1. The extent to which the firm has resources and personnel with the necessary experience and
training to perform the services as outlined herein. (25 pts)
2. Knowledge of local, state and federal regulations regarding safety compliance, employee
health and safety, specifically, Cal-OSHA. (20 pts)
3. Experience and references. (15 pts)
4. The extent to which the firm can provide the required services as listed under Major
Responsibilities. (30 pts)
5. The extent to which the Provider's proposal demonstrates a thorough understanding of the
scope of the undertaking. (15 pts)
6. Cost compared to services provided. (20 pts)
7. Ability to provide services in a convenient location with flexible hours of availability for
injured workers. (15 pts)
EVALUATION CRITERIA
Proposals submitted will be subject to the City's selection procedures for technical and/or
professional consultants. All proposals will be initially reviewed to determine if they are
responsive to all of the City's requirements. Those proposals deemed non-responsive may be
eliminated from consideration. Accordingly, final selection will be based upon overall capability
to perform services and not exclusively upon cost of services. Final selection may be made upon
completion of an oral interview and reference checks of top scoring Consultants. The City of
Redding reserves the right to award the Employment Physicals and the Treatment of Industrially
Injured Workers separately.
INDUSTRIAL INJURY TREATMENT
Industrial injury treatment will be billed and paid at the Official Medical Fee Schedule (OMFS),
or an agreed upon discounted PPO agreement, as may be applicable. The OMFS is promulgated
by the California Division of Workers' Compensation administrative director under California
Labor Code section 5307.1 and can be found in sections 9789.10 et seq. of Title 8, California Code
of Regulations. See the State's website at www.dir.ca.gov/dwc/OMFS9904.htm. The City
reserves the right to obtain a third party for the purpose of bill review to ensure fees are in
compliance with state and local schedules.
PAYMENTS
Payment will be issued to the successful consultant upon receipt of billing indicating the specific
services provided, dates of service, place of service, etc., and subject to the Fee Schedule as
outlined in the Consulting and Professional Services Agreement.
Invoices for industrial injury treatment are to be provided on forms and in the format as outlined
in the California Labor Code. All industrial injury treatment invoices will have an attached
treatment report.

CITY OF REDDING
CONSULTING AND PROFESSIONAL SERVICES CONTRACT
THIS CONTRACT ("Contract") is made at Redding, California, by and between the City of
Redding ("City"), a municipal corporation, and _________________ ("Contractor") (collectively
the "Parties, individually a "Party") for the purpose of _______________.
WHEREAS, City does not have sufficient personnel to perform the services required herein
thereby necessitating this Contract for personal services.
NOW, THEREFORE, the Parties covenant and agree, for good consideration hereby
acknowledged, as follows:
SECTION 1. CONTRACTOR SERVICES
Subject to the terms and conditions set forth in this Contract, Contractor shall provide to
City the services described in Exhibit A, attached and incorporated herein. Contractor shall
provide the services at the time, place and in the manner specified in Exhibit A.
SECTION 2. COMPENSATION AND REIMBURSEMENT OF COSTS
A. City shall pay Contractor for services rendered pursuant to this Contract, at the times
and in the manner set forth in Exhibit B, attached and incorporated herein, in a total
amount not to exceed $_____. This sum includes all out-of-pocket travel, lodging
and incidental expenses incurred by Contractor that are reasonably associated with
the provision of services under this Contract. The payments specified herein shall be
the only payments to be made to Contractor for services rendered pursuant to this
Contract.
B. Contractor shall submit monthly invoices to City for work completed to the date of
the invoice. All invoices shall be itemized to reflect the employees performing the
requested tasks, the billing rate for each employee and the hours worked.
C. All correct, complete and undisputed invoices sent by Contractor to City shall be paid
within thirty (30) calendar days of receipt.
SECTION 3. TERM AND TERMINATION
A. Contractor shall commence work on or about _______________ and complete said
work no later than _______________. Time is of the essence.
B. If Contractor fails to perform its duties to the satisfaction of City, or if Contractor
fails to fulfill in a timely and professional manner its obligations under this Contract,

then City shall have the right to terminate this Contract effective immediately upon
City giving written notice thereof to Contractor.
C. Either Party may terminate this Contract without cause on thirty (30) calendar days'
written notice. Notwithstanding the preceding, if the term set forth in Section 3.A.
of this Contract exceeds ninety (90) calendar days in duration, Contractor's sole right
to terminate shall be limited to termination for cause.
D. Contractor hereby acknowledges and agrees that the obligation of City to pay under
this Contract is contingent upon the availability of City's funds which are
appropriated or allocated by the City Council. Should the funding for the project
and/or work set forth herein not be appropriated or allocated by the City Council,
City may terminate this Agreement by furnishing at least thirty (30) calendar days'
written notice of its intention to terminate. In the event of a termination pursuant to
this subdivision, Contractor shall not be entitled to a remedy of acceleration of
payments due over the term of this Agreement. The Parties acknowledge and agree
that the power to terminate described herein is required by Article 16, Section 18, of
the California Constitution, and that constitutional provision supersedes any law,
rule, regulation or statute which conflicts with the provisions of this Section.
E. In the event that City gives notice of termination, Contractor shall promptly provide
to City any and all finished and unfinished reports, data, studies, photographs, charts
or other work product prepared by Contractor pursuant to this Contract. City shall
have full ownership, including, but not limited to, intellectual property rights, and
control of all such finished and unfinished reports, data, studies, photographs, charts
or other work product.
F. In the event that City terminates the Contract, City shall pay Contractor the
reasonable value of services rendered by Contractor pursuant to this Contract;
provided, however, that City shall not in any manner be liable for lost profits which
might have been made by Contractor had Contractor completed the services required
by this Contract. Contractor shall, not later than ten (10) calendar days after
termination of this Contract by City, furnish to City such financial information as in
the judgment of the City's representative is necessary to determine the reasonable
value of the services rendered by Contractor.
G. In no event shall the termination or expiration of this Contract be construed as a
waiver of any right to seek remedies in law, equity or otherwise for a Party's failure
to perform each obligation required by this Contract.
SECTION 4. MISCELLANEOUS TERMS AND CONDITIONS OF CONTRACT
A. City shall make its facilities accessible to Contractor as required for Contractor's
performance of its services under this Contract, and, upon request of Contractor,
provide labor and safety equipment as required by Contractor for such access.

B. Pursuant to the City's business license ordinance, Contractor shall obtain a City
business license prior to commencing work.
C. Contractor represents and warrants to City that it has all licenses, permits,
qualifications and approvals of any nature whatsoever that are legally required for
Contractor to practice its profession. Contractor represents and warrants to City that
Contractor shall, at its sole cost and expense, keep in effect or obtain at all times
during the term of this Contract any licenses, permits and approvals that are legally
required for Contractor to practice its profession.
D. Contractor shall, during the entire term of this Contract, be construed to be an
independent contractor and nothing in this Contract is intended, nor shall it be
construed, to create an employer/employee relationship, association, joint venture
relationship, trust or partnership or to allow City to exercise discretion or control over
the professional manner in which Contractor performs under this Contract. Any and
all taxes imposed on Contractor's income, imposed or assessed by reason of this
Contract or its performance, including but not limited to sales or use taxes, shall be
paid by Contractor. Contractor shall be responsible for any taxes or penalties
assessed by reason of any claims that Contractor is an employee of City. Contractor
shall not be eligible for coverage under City's workers' compensation insurance plan,
benefits under the Public Employee Retirement System or be eligible for any other
City benefit.
E. No provision of this Contract is intended to, or shall be for the benefit of, or construed
to create rights in, or grant remedies to, any person or entity not a party hereto.
F. No portion of the work or services to be performed under this Contract shall be
assigned, transferred, conveyed or subcontracted without the prior written approval
of City. Contractor may use the services of independent contractors and
subcontractors to perform a portion of its obligations under this Contract with the
prior written approval of City. Independent contractors and subcontractors shall be
provided with a copy of this Contract and Contractor shall have an affirmative duty
to assure that said independent contractors and subcontractors comply with the same
and agree to be bound by its terms. Contractor shall be the responsible party with
respect to all actions of its independent contractors and subcontractors, and shall
obtain such insurance and indemnity provisions from its contractors and
subcontractors as City's Risk Manager shall determine to be necessary.
G. Contractor, at such times and in such form as City may require, shall furnish City
with such periodic reports as it may request pertaining to the work or services
undertaken pursuant to this Contract, the costs or obligations incurred or to be
incurred in connection therewith, and any other matters covered by this Contract.
H. Contractor shall maintain accounts and records, including personnel, property and
financial records, adequate to identify and account for all costs pertaining to this
