RFP for Case Management Services with Mental Health Focus

The purpose of this Request for Proposals (RFP) is for Washington County Community Services to expand Contracted Case Management services funded by home and community-based waiver programs, as well as Vulnerable Adult and Developmental Disability Targeted Case Management (VA/DD TCM) services and County funded Rule 185 Case Management Services.

Additionally, we are seeking waiver case management Service Providers that also specialize in providing related mental health services, particularly those interested in providing contracted case management with a mental health focus. Our goal is to enhance contracted case management service delivery within the community by partnering with providers who can effectively support individuals with mental health diagnoses, that also qualify for one of the following home and community based waivers: Community Access for Disability Inclusion (CADI), Brain Injury (BI), Elderly (EW) or Developmental Disability (DD) waiver services.
