RFP Name: Employee Assistance Program (EAP) Benefits for Los Alamos County
Only one of the following submission methods is required:
Sealed proposals in one (1) clearly labeled unbound original, and three (3) paper copies, and one (1) electronic version on a USB flash drive or CD, will be received at the Office of the Chief Purchasing Officer, Procurement Division - 101 CAMINO ENTRADA, BUILDING 3, Los Alamos, NM 87544, until 2:00 p.m. Mountain Time, August 8, 2023 for this solicitation.
Proposals may also be submitted electronically by email in pdf format. All other requirements stated in the Solicitation Document remain in effect. Emails should be addressed to:

CRITICAL SUBMISSION OF LETTER OF TRANSMITTAL. DUE NO LATER THAN 2:00 P.M. MOUNTAIN TIME, JULY 14, 2023.
Due to the sensitive, confidential nature of some information to be disclosed for preparation of this RFP, a signed Letter of Transmittal is required before census, claim data, or other such sensitive or confidential information will be released to potential Offerors. Only carriers deemed by the Project Manager to be qualified to provide the Scope of Services listed in this RFP and who complete and accurately submit a signed Letter of Transmittal to the recipients specified below, by the specified deadline, will be eligible to receive this information. Only proposals received by those carriers who have successfully completed this requirement shall be evaluated.
The Letter of Transmittal document Offerors are required to use is included as Exhibit E of this RFP Document.
