RFP-2026-DMS-04-COMMU: Community Reentry Navigator and Communications Services

The New Hampshire Department of Health and Human Services, Division of Medicaid Services (“Department”) is seeking responses to this Request for Proposals ("Solicitation" or "RFP") from qualified Vendors to provide navigator services to support correctional facilities and incarcerated individuals. These services will include assistance with initial Medicaid enrollment under the 1115 demonstration benefit and youth reentry benefit for prelease services, applications for other health insurance coverage, preparation for community reentry, and transition out of the Medicaid program as needed. For individuals who remain eligible for Medicaid post-release, the selected Vendor will provide support to maintain compliance with community engagement requirements. Additionally, the selected Vendor will develop and disseminate educational materials to inform both incarcerated individuals and the general public about the community reentry program.
