Bids are accepted for the total re-roof one duplex located at 420 and 422 Queets Avenue in
Queets, Wa, the scope of work is as follows;
Remove and dispose of existing roofing
Remove and replace underlayment
Install metal Snap-Lok standing seam metal roofing, same color as existing
Install new vents and flashings
Install new pipe boots
Install vented ridge cap
Install drip edge
Install gable metal
Remove and replace roof to wall metal flashing
Remove and replace fascia and barge board and prime white
Remove and replace gutters and downspouts, same color as existing
I. Indian Preference (20 pts)
The Quinault Indian Nation has enacted a TERO Ordinance governing Indian Preference,
and the Quinault Housing Authority has adopted an Indian Preference Policy, set forth in
section 3000.10 of the QHA Procurement Policy. To the greatest extent feasible,
preference shall be given to Indian organizations and Indian-Owned economic enterprises
in the award of all contracts and subcontracts.
A. An Indian, Indian –Owned economic enterprise or Indian organization is eligible for
Indian Preference (15 points)
B. Proposals shall include a statement describing how you will provide for Indian
preference in subcontracting, training and employment in this contract, including
the number or percentage of Indians to be employed and trained, in accordance
with Section 3000.10 of the QHA Procurement Policy. All proposals must include
this statement. (5 points)
Contractors applying for eligibility for Indian preference shall submit the following:
a) Evidence showing the applicant is an Indian of a federally recognized Indian tribe. A
certificate of Indian blood or census card will suffice.
b) Evidence showing fully the extent of Indian ownership and interest.
II. Price (80 pts)

The contractor selected must have all required insurance, including worker’s
compensation, general liability insurance of not less than $500,000.00 per occurrence
and $100,000.00 aggregate.
Contractor selected will be required to complete a TERO compliance plan and pay the
1.75% TERO fee; per QTC 97.03.040 (d) the TERO fee cannot be included in the bid price.
The contractor will be required to follow all TERO regulations; including hiring from the
TERO hall, if required by the TERO office. If the project is to be completed in five days or
less, please state that in the bid. Projects for this duration are exempt from TERO
requirements.
Contractor selected will be required to possess a QIN business license.
All proposals must include the name of all owners of the company.
All proposals must include a completed Form HUD 5369C
The contractor selected must submit a W-9
Unless contractor is a sole proprietorship Workman’s Compensation coverage will be
required
As a housing authority in Washington State QHA is exempt from all taxes.
Any required permits to be obtained by the contractor.
To the greatest extent practicable preference shall be given for the use of materials
produced in the United States.
If the total contract price is $25,000 or higher, a 5% bid bond must accompany the bid.
Site visits are mandatory.
