Request for Bids Rehab
The Quinault Housing Authority (QHA) is seeking bids for the re-leveling of 1 two story house
located at 319 Cuitan Street, Taholah Wa.
Provide a detailed proposal for how the house will be re-leveled and the total cost to perform
the work.
Where “equivalent” or “equal” is used in the scope of work the contractor must submit
specifications to show that it is an equivalent product and receive approval, in writing, from
QHA prior to installation.
Contractors are required to perform a site visit prior to submitting a bid.
I. Indian Preference (20 pts)
The Quinault Indian Nation has enacted a TERO Ordinance governing Indian Preference,
and the Quinault Housing Authority has adopted an Indian Preference Policy, set forth in
section 3000.10 of the QHA Procurement Policy. To the greatest extent feasible,
preference shall be given to Indian organizations and Indian-Owned economic enterprises
in the award of all contracts and subcontracts.
A. An Indian, Indian –Owned economic enterprise or Indian organization is eligible for
Indian Preference (15 points)
B. Proposals shall include a statement describing how you will provide for Indian
preference in subcontracting, training and employment in this contract, including
the number or percentage of Indians to be employed and trained, in accordance
with Section 3000.10 of the QHA Procurement Policy. All proposals must include
this statement. (5 points)
II. Price (80 pts)
Provide total cost to complete the project.
Other provisions
• The contractor selected must have all required insurance, including worker’s
compensation, general liability insurance of not less than $500,000.00 per occurrence
and $100,000.00 aggregate.
• Contractor selected will be required to complete a TERO compliance plan and pay the
1.75% TERO fee; per QTC 97.03.040 (d) the TERO fee cannot be included in the bid price.
The contractor will be required to follow all TERO regulations; including hiring from the
TERO hall. Any project performed in less than five days is exempt from all TERO
regulations.
• Project is subject to Tribal Prevailing Wage Rate.
• Contractor will be required to possess a QIN business license by the start of the project.
• All proposals must include a completed Form HUD 5369C
• The contractor selected must submit a W-9 if QHA has not worked with you before.
• Unless contractor is a sole proprietorship Workman’s Compensation coverage will be
required.
• As a housing authority in Washington State QHA is exempt from all taxes.
• Permit to be obtained by the contractor.
• To the greatest extent practicable preference shall be given for the use of materials
produced in the United States.
