Division of Long Term Supports and Services
The New Hampshire Department of Health and Human Services, Division of Long Term Supports and Services (“Department”) is seeking responses to this Request for Applications ("Solicitation" or "RFA") from qualified Vendors to provide housing navigation services to individuals participating in the Money Follows the Person (MFP) program who are seeking to return to a community-based residence from an institutional setting.
