REQUEST FOR PROPOSAL

AWARD OF CONTRACT

The District anticipates multiple contract awards. Any proposal that achieves a minimum total score of 60 points will receive an award.

You must submit a statement that the product you are proposing is the same product as in the original solicitation so that we do not need to request a sample and do another taste testing.

Vendor must be able to provide velocity reports as requested.

The purpose of this Request for Proposal (RFP) is to solicit competitive sealed bids from qualified suppliers for procurement of processing USDA commodity mozarella cheese for bread sticks, lasagna roll ups, manicotti and ravioli.

Please make sure to include in your proposal Commercial pricing that is located on the costsheet attached. If our USDA allotment is inadequate for our yearly needs and we need to purchase an identical product at commercial pricing.

Please read the RFP closely making sure that your response includes all, completed, required documentation.

Please see attached specification document for further instructions and information.

A vendor/person with an outstanding tax lien in the State may not submit a quote, bid or offer.

We have made positive efforts to involve minority and small business.

We anticipate the value of this contract for a year to be $400,000.
