REQUEST FOR INFORMATION
Heartland Whole Health Institute is now accepting Requests for Information from implementation partners for a large-scale health care initiative designed to transform how chronic cardiometabolic diseases are prevented, managed, and reversed.
Organizations and vendors that offer any of the following services are encouraged to respond: digital and human-enabled health coaching; behavioral and mental health; "food-as-medicine" nutrition programs; community wellness initiatives; and social drivers of health navigation.
The RFI process opens November 2 and closes November 14, 2025. A full version of the RFI, including scope of services and other information, is available on https://www.heartlandwholehealth.org/research-advocacy/.
