The City of Fremont Fire Department is seeking a qualified medical provider to provide NFPA 1582 complaint annual physical examinations to members of the City of Fremont Fire Department. The medical provider response to this Request for Proposals (RFP) to furnish all labor and equipment in providing NFPA 1582 compliant physical.
The firm should submit one (1) original and one (1) hard copy. Proposals must be submitted and received in the Fremont City Clerk’s Office by the date and time listed below.
Final proposals are to be submitted by Thursday, October 9th, 2025 no later than 2:00 p.m. to the City Clerk’s Office, 400 East Military Ave., Fremont, NE 68025.
PROPOSALS MAYBE REJECTED IF NOT IN COMPLIANCE WITH THESE REQUIREMENTS.
1. Proposals are to be received by the date and time of the proposal opening indicated above.

2. Proposals must be in a sealed envelope/container and marked “RFP – Fire Dept Medical Exams”. Container(s) utilized for original documents must be clearly marked ORIGINAL DOCUMENTS.
3. The City shall not be liable for any pre-contract costs incurred by interested providers participating in the selection process.
4. The City reserves the right to reject any and all quotes or any part thereof, or to accept any quote or any part of thereof, deemed to be in the best interest of the City.
5. The contents of each firm’s proposal shall remain valid for a minimum of ninety (90) calendar days from the Proposal due date.
