1 Scope of Work
The City of Lake Forest is soliciting proposals from qualified providers of Verkada card readers and door
controller hardware, whose product offerings meets or exceeds current City needs as identified in 'Desired
Functionality'. The City is in the first phase of implementing a new access control system that will be rolled out
in phases throughout the City in the upcoming years. The City reserves the right to award future phases at a
later date, based on project funding and schedule. The City reserves the right to initiate a separate future
phase RFP subject to project funding and schedule.
1.1 Desired Functionality
The Desired Functionality section is intended to identify functionality that the City would require for
replacement and additions to our Municipal Service's access control system. The City currently has a large
Verkada footprint that allows for better integration and analytics for security purposes. The City's IT
Department feels it is in the City's best interest to move forward with the Verkada platform with access control
as well. The hardware listed out is explicitly Verkada access control hardware, unless specified otherwise.
Please include labor cost for the installation of the following hardware.

Professional Services for Configuration
* Successful Proposer will provide hardware installation and network supplementation is anticipated to be
completed by City IT staff.
* Site survey will be conducted prior to final contract approval to determine final hardware counts
Licensing and Maintenance
* Provide pricing for one year of usage licensing, 8x5 support and maintenance of the recommended
hardware.
* Must include annual web-based administration and analytics in pricing.

Submission Procedures
Intent to Propose
Prospective Vendors are encouraged to submit an intent-to-propose email notification. See Proposal
Submission Summary on page 2 for deadline and recipient. Submitting an intent-to-propose document does
not obligate the Vendor to submit a proposal and is not mandatory to submit a proposal.
RFP Clarifications and Questions
If any person contemplating submitting a proposal is in doubt as to the true meaning of any part of this RFP,
they may submit to the City a written request for an interpretation thereof.
See Proposal Submission Summary on page 2 for deadlines and recipient. Responses to questions will be
emailed to all Vendors who have submitted a timely intent to propose communication.
Proposal Evaluation Process
Responses to this RFP will be evaluated by a City Selection Team consisting of various stakeholders from
multiple departments. At any point in time during the evaluations a Vendor may be excluded from further
consideration. Reference checking with comparable entities using the Proposer's product will be conducted
after Proposal submission. The top Proposers may be asked to provide a system/hardware demonstration
and/or professional services interview.
Proposal
To be considered responsive, Proposals must be submitted by the due date and time and contains an original
signature of a company officer on the General Certification Requirements Form.
To facilitate the analysis of responses to this RFP, the Proposer is required to prepare their proposals in
accordance with the instructions outlined in this section. The proposal should be organized as follows:
Executive Summary
This part of the response to the RFP should be limited to a brief narrative not to exceed two (2) pages
describing the Vendor's ability to provide the City's solicitation identified in this RFP. Vendors must provide
information about their company so that the City can evaluate the Vendor's stability and ability to support the
commitments set forth in response to this RFP.

Proposed Services
The requirements defined in 'Scope of Work' contain the overall desired outcomes of the requested goods
and/or services. Each Vendor should review the desired services/scope of work requested and respond as to
their availability.
Cost
* Please provide costs for the hardware count in the Desired Functionality table, training, and 1 year of
maintenance.
* Do not use, "To Be Determined," "Time and Materials," or similar annotations, as the City is asking
Proposers to provide pricing for all desired outcomes with the understanding that they may have to
make assumptions. Such assumptions should be stated in this section.
* All on-going or recurring costs must be provided
* Vendors may submit multiple versions of pricing forms to reflect multiple proposal options, if offered.

Contract Terms & Conditions
The contract will be fully negotiated after a Vendor's proposal is accepted by the City. Responses to the
contract terms outlined in this section will be considered in the selection process and such terms will be
considered part of the final contract.
Insurance
The following requirements are baseline standards for insurance to be provided. They may require adjustment
from time to time based on a variety of factors, including the nature, scope, duration, and value of the
procurement, and are subject to approval of the City.
Comprehensive General Liability
With coverage written on an "occurrence" basis and with limits no less than:
$2,000,000 Bodily Injury and Property Damage Combined Single Limit
Coverage is to be written on an "occurrence" basis.
Coverages shall include:
- Broad Form Property Damage Endorsement
- Blanket Contractual Liability (must expressly cover the indemnity provisions of the Contract)
- Independent Contractors
- Personal Injury (with Employment Exclusion deleted)
- Broad Form Property Damage Endorsement
- Bodily Injury and Property Damage
Professional Liability Insurance
With a limit of liability of not less than $2,000,000 per occurrence and $2,000,000 in the aggregate and covering
the consultant's errors and omissions.
Pricing Eligibility Period
All Vendor proposals are required to be offered for a term not less than 90 calendar days in duration. A
proposal may not be modified, withdrawn or cancelled by the Vendor during the 90-day time period following
the time and date designated for the receipt of proposals.
