SOURCES SOUGHT



BACKGROUND:



The Albuquerque Area Indian Health Service (AAIHS), Santa Fe Indian Health Center (SFIHC) Optometry Department has a requirement for replacement of Optometry Equipment (See Below).



OBJECTIVE:



The Contractor shall provide and ensure that all medical equipment and systems fit into and be compatible to work with the existing floor, location, framework, electrical, and plumbing systems of the existing medical facilities and departments specified in this scope. The Contractor shall provide the following:




  1. Reliance 7900 IC Floor Unit with console or equal. The floor stand shall include and meet the following: Model – 7900 IC, Power – 115V AC 60Hz 3.0A, Weight – 292 lbs., Warranty – 1 year, Height to Console – 36 inches, Height to Pole Top – 71 inches, Base – 21 in x 21 in, Membrane switches, Overhead halogen lamp.

  2. Reichert 11636 W/ Minus Cylinder or equal. The phoroptor shall include and meet the following: Model: 116 with minus cylinder. Power: 115V 50/60 Hz 100 mA, Weight: 9.5 lbs., Height: 11.56 inches, Width: 12.75 inches, Depth: 3.90 inches, Warranty: 1 year, Included Accessories: ±0.25 flip cross cylinder, +1.50 Retinoscopic lenses, Sphere: -19.00 to +16.75D, 0.25D Steps, +0.12D Sphere in auxiliary dial. Cylinder Power: 0.00D to 6.00D, 0.25D steps, Plus or minus lens system optional 0.12D and 2.00D accessory cells. Cylinder Axis: 360 Degrees (Double 0 to 180) and 5 degree graduations. Cross Cylinder: ±0.25, ±0.37 and ±0.05 optional. Prism: 0 to 20 Prism diopter with 1 prism graduations. Pupillary Distance: 48mm to 75mm with 1mm graduations. Vertex Distance: 13.75mm and graduations extend range by 6mm. Reading Distance: 5 to 28 inches, included centimeter and diopter scales. Auxiliary Dial: 10 lenses plus 2 open apertures.

  3. Haag-Streit BP900. The slit lamp shall include and meet the following. Model: BP 900, Power: 115V 50/60 Hz, Height: 27.6 inches, Width: 12.25 inches, Depth: 12 inches. Slit Width: 0 to 14 mm continuous, Slit Length: 1 to 14 mm continuous and fixed apertures, Magnification: 10x, 16x and 25x. Slit Angles: ±90 Degrees, Filters: Blue, Red-free (Green) and Grey (10%). Light Source: LED. Diopter Adjustment: +6 ro -6 diopters.

  4. Haag-Streit BP/BQ/BX Applanation Tonometer. The tonometer shall include and meet the following: Model: AT 900. Power: 115V 50/60 Hz. Weight: 1.08 lbs. Warranty: 1 year. Measurement Range: 0 to 80 mm Hg. Measurement Divergence: 0 to 58.84 mN, force impingement on the measuring prism over a measuring range. Standard Divergence: 0.49 mN ≤ 3 s ≤ 1.5 % of rated value. Reverse Span: ≤ 0.49 mN.



OTHER REQUIREMENTS:



Trade-In/Other: Contractor must pick up and dispose of existing equipment and provide trade in pricing.



Training Support: Contractor must provide training on new equipment after installation.



Training Support: Contractor must provide training on new equipment after installation.



Warranty: Contractor must provide an equipment warranty plan for the duration of the contract.



Compatibility w/EQP or Software: Documentation must be included with quote detailing equipment compatibility with:

Customer Support: Contractor must provide 24/7 customer support for the duration of the contract.



Emergency Support: Contractor must provide 24/7 emergency support and respond to emergency support request within 4 hours.



Energy Consideration: Equipment must be energy star rated.



Environmental Considerations: Equipment must be water efficient.



Maintenance: Contractor must provide a maintenance plan for the duration of the contract.



HOURS OF OPERATION & PLACE OF DELIVERY:



All deliverables shall be delivered to the client no later than the date specified in the SOW unless otherwise agreed upon. Hours of installation will be M-F from 8:00 AM to 4:00 PM.



The Vendor shall include all associated shipping costs and provide shipping date information to the Contracting Officer’s Representative (COR) or Biomed listed. The equipment shall be shipped to the following location:

STATEMENT OF OBJECTIVES:




  • Locate eligible and qualified Small Business Indian Firms to support IHS procurement needs under NAICS code: 333310 – Commercial and Service Industry Machinery Manufacturing.

  • Utilize the information received with future acquisition planning efforts and potential Buy Indian set aside.



HOW TO RESPOND:



This is a sources sought notice under the authority of the Buy Indian Act. The Indian Health Service (IHS) is attempting to locate qualified and eligible Small Business Indian Firms, “Native American Indian Owned and Operated businesses,” that can provide services/supplies to the Indian Health Service. This is a request for information only, this is not a solicitation. Responses to this notice are not considered offers and cannot be accepted by the Government to form a binding contract. Any information submitted by respondents to this notice is strictly voluntary with no obligation or incurred cost by the government.

  1. E-mail response. Shall be in a printer friendly format, Word or PDF, 8x11 page size, 5-page limit.

  2. Certification as a Small Business Indian Firm. The government is ONLY interested in Small Business Indian Firms. (See attached Representative Form).

  3. The government requests that interested parties respond to this notice if applicable and identify your business status to the identified NAICS code.

  4. Prospective vendors shall be properly registered in SAM.gov; this includes proper registration as a Small Business Indian Firm and if eligible, registered as applicable small business certifications (e.g. HubZone, 8(a), Service Disabled Veteran Owned, Veteran Owned, Women-Owned, Small Disadvantaged), in order for IHS to issue an award and claim procurement actions with eligible Small Business Indian Firms.

  5. Provide detailed capabilities statement, website, and e-catalog as it pertains to 333310 – Commercial and Service Industry Machinery Manufacturing.

  6. Provide Unique Entity Identifier (UEI) created in SAM.gov registration.
