REMOVAL OF EQUIPMENT WASTE

Project Description: Remove and dispose of miscellaneous bulk equipment, furniture and other items from
the John A. Burns School of Medicine at 651 Ilalo Street, Honolulu, Hawaii 96813.

Bid must include all applicable charges, taxes, shipping, etc. Bid price shall be
considered firm.

Bid price shall be valid for a minimum of 45 days.
PLEASE SUBMIT YOUR QUOTATION BY NO LATER THAN 2:00 PM on January 9, 2026.

Vendors who attach additional documents (quotations, proposals, etc.) with terms and
conditions shall be disqualified.

Under Hawaii law, HRS 103D-310(c), you must provide proof of compliance in order to
receive a contract worth $2,500 or more with state and county government entities in
Hawaii.

Vendors need to obtain a copy of their \"Certificate of Vendor Compliance\" via Hawaii
Compliance Express. To register with Hawaii Compliance Express, go to
https://vendors.ehawaii.gov/hce/splash/welcome.html. Vendor must provide proof of
compliance 1 day after closing date.
