Description
The District is requesting proposals from qualified vendors to provide a Remote Patient Monitoring solution with the following goals:
- Improve patient outcomes and early intervention.
- Implement a remote monitoring platform with continuous monitoring capabilities and configurable alert thresholds.
- Integrate the platform with existing JPS systems, including the electronic health record and other approved operational systems, as appropriate.
- Deploy dashboards, alert workflows, escalation pathways, and role-based work queues to support timely intervention.
- Provide implementation support, training, device logistics guidance, and post-go-live optimization.
