Description

The Housing Suppo rt Contractor should utilize the Scope of Works utilizing the policies of the

CRNP and the Houston Health Department regarding the performance requirements to provide

housing support services to CRNP participants in the designated Houston, Harris County, Texas

area. The terms of this service shall be from the date awarded through September 30, 2027.


The Vendor shall:

Provide service(s) to Community Reentry Network Program (CRNP) assignees authorized by the

Houston Health Department (HHD) staff as identified below, in accordance with the vendor

application, funding requirements, all required assurances, licenses and certifications as

applicable.

Provide Housing Support including:
