Summary of Work:
Base Bid: All work associated with roofs 11, 12 and 13 including concrete panel replacement.
All other work from first bid is now shown as alternates.
Oher changes from first bid:
- Liquidated damages reduced to $2,000
- Changes to contract language
- All addendum questions incorporated either by word on drawing A-1 or by change of
specifications.
A pre-bid conference will NOT be held for this re-bid.

Bidders are responsible for filling out the required UBE Construction forms and including them

writing via the email below according to the directions given in the Request for Approval of
Substitution and in the Instructions to Bidders prior to 3pm, on Monday, April 8th, 2024.

Bid Forms and Other Requirements:
Each bidder must show evidence that they are licensed under Chapter 87 of the N.C. General
Statutes. A General Contractors License is required.
The following forms MUST be filled out and included with the bid proposal:
1. Bid Forms
2. Non-Collusion Affidavit
3. Contractor Safety Record Information
4. Contractor Workforce Diversity Questionnaire
5. Bid Bond
6. Equal Business Opportunity Program Construction Forms:
a. Form E-101 (UBE Participation On Base Bid), and
b. Form E-102 (UBE Participation On Bid Alternate)
Any bids submitted without these forms may be deemed as non-responsive.
The City of Durham reserves the right to reject any or all bids, waive informalities, and/or
accept such bid as appears in its judgement to be in the best interest of the City.
