Attachment 2
STATEMENT OF WORK (SOW)
Background
The VBA Muskogee Regional Office (VARO 351) has a need for Personal Assistant Services (PAS) services for an employee due to a Reasonable Accommodation.
Scope of Work
Contractor to provide PAS services for the entire workday (8AM-8PM) approximately 2080 hours per year to include the following:
Assists with feeding/drink.
Assists with bathroom duties.
Needs to be able to quickly change catheter if there are problems.
Administers medication.
Helps provide relief and deal with muscle spasms throughout the day.
Repositions him as needed.
Helps reset devices when needed.
General unforeseen issues that arise.
Drive wheelchair converted van.
Provide all necessary support during the scheduled workday to include the commute to and from the work location.
