Based on the description on page 4 and 5 a Regular size car or SUV should be able to handle the size specifications but there is no mention anywhere of the quantity of boxes to be picked up on each stop? It does state on the pricing section we may have 3 different pickups and delivery: lab samples, medical supplies and container to be addressed, however does not states where these are to be picked up from. The contractor will determine vehicle size; however, 1-2 containers at each location of lab items with the other on an as needed basis based off patient/clinician demand. Quantity of boxes at each location - There will be 1-2 containers for lab pick up at each location - Surgical instruments are as needed - Medical supply item is as needed Pickup Location: - Lab and surgical instrument at Rosedale, Fort Meade, Glen Burnie are in a central location depending upon facility - Lab and surgical instrument at Loch Raven CLC and Loch Raven Clinic are in multiple locations - Medical Supply items o Perry Point Lab for as needed delivery to Pocomoke and Cambridge o Baltimore Lab for as needed delivery to Loch Raven CLC, Loch Raven Clinic, Rosedale, Fort Meade, Glen Burnie 2) On page 7 of the description of services on item # 0002 is that the work that is described on page 5 section A. Eastern shore? Service item #0002 is described on page 5 as the Eastern Shore 3) on page 8 item# 0003 is that work that is described on page 5&6 section B Greater Baltimore? Service item #0003 is described on page 5 as Greater Baltimore 4) On page 8 item 0004 that seems the same description as item #0003 and we do not see the work on page 6 section C Loch Raven Medical anywhere on the price/cost schedule? What happens to that work could that be a mistake and should be the item #0004 and if so is Loch Raven clinic and CLC at the same address as there are different time of pick up for each section? The difference between #0003 and #0004 is error and amendment forthcoming 5) on page 5 & 6 we have pickups that are scheduled at 4 PM from Fort Mead, Glen Burnie, Loch Raven and Rosedale there is no way we can deliver these by 5 PM to VAMC, it can be done but traffic could play a part and for sure fort Meade will be the hardest to make the 5 pm deadline of VAMC. Pickup time is essential; the estimated transportation time from Rosedale, Glen Burnie, Loch Raven, Fort Meade is under 60mins; however, occasional delays are permitted provided they do not exceed 90 mins. 6) Is VAMC location mentioned on pricing 10 North green street Baltimore? VAMC Baltimore 10 N. Green St Baltimore, MD VAMC Perry Point Bldg. 361 Perry Point, MD 7) In description of page 5 section 1e we should assume that these deliveries are round trip? on the pricing section cost schedule it states to and from should we rate these as round trips and if round trip what is the dead line for getting it back to the original pick up point? Deliveries are to and from or round trip, it is up to the contractor to determine the path of travel and route for efficiency. Pickup times are listed on page 5 and 6 at each location with the expectation the delivery to VAMC Baltimore or VAMC Perry Point within the allotted time frame. 8) beside these scheduled runs is there any on demand and emergency deliveries during the work hour. If you, please add that to the questions we appreciate it. No additional requirement for on demand and emergency deliveries during the work hours. Do not anticipate any STAT runs. Any emergency (STAT) runs are internally coordinated between Physician and Lab Staff. 9) Item Numbers 0001, 1001, 2001 & 3001 Please provide details on the Friday delivery of instruments from Cambridge CBOC to Baltimore VAMHCS including all pickup times and delivery times. Friday pickup for Cambridge CBOC is at 4pm on Friday for delivery to VAMC Baltimore. 1-2 small containers no larger than 15 x 15 x 15 inches, weighing under 20 pounds. 10) Item Numbers 0004, 1004, 2004 & 3004 Appear to be a duplicate of Item Numbers 0003, 1003, 2003 & 3004. Short term contract $71,250.00 no options 14) What type of vehicles are currently being used to transport the shipping containers; passenger vehicles, vans, SUV, etc.? Vehicle and drivers are determined by the contractor 15) How many drivers are currently being used to provide the listed services? Vehicle and drivers are determined by the contractor 16) What is the maximum number of containers picked up at largest/busiest CBOC? Maximum is 4 containers Each location will have 1-2 Lab containers Instrument and supply containers are dependent upon clinical schedule 17) Will containers be available for pickup in advance of the listed pickup times? Surgical instrument and medical supply containers 18) 4. Will dry ice be needed for specimen transport? Is it the responsibility of the contractor to provide dry ice? Lab specimens are packaged by each facility (Lab or Nursing staff member) using reusable cold/ice packs. 19) Regarding specific personnel and vehicles to perform and their documentation: It is common practice for small businesses that the drivers, cars and other equipment used for a contract are not sourced until after an award has been made, for cost effectiveness and to reduce loss. Will you be accepting this documentation after the award? Since personnel and vehicles are apart of technical evaluations. You would need to submit documentation when you submit your quote. 20) Regarding evaluation factors for the price Volume: we usually like to provide a Pricing realism cost break-down, to include SCA minimum per year, the number of drivers, hours and miles that are estimated to be required for performance, and other costs. Is it acceptable that we forward this Pricing Realism chart with our Price response? Pricing Realism chart is fine to submit with quote 21) Will this contract be SCA enforced? After reviewing the solicitation on FBO, it contains language and dollar values regarding the Service Contract Act. Can you verify that the minimum required wage will be $16.04, and H&W of $4.22? Yes , and the information is correct 22) SBA Final Rule regarding FAR Clause 52.219-14 Limitations on Subcontracting, dated June 30, 2016, reclassifies Independent Contractor drivers as Subcontractors 13 CFR 125.6 Work performed by an independent contractor shall be considered a subcontract. Please inform us how the VA would like us to proceed upon award regarding this rule change given that it will affect our offer regarding pricing and performance. 50% of the work have to be done by the SDVOB and sub-contractor have to be a SB. 23) VAMC closes at 5 PM the 4 PM pickups could be tricky to make it by 5 PM with our area traffic what happens if we are running late? Call the pickup or drop-off location. 24) Are these runs one way or round trip (section 1e) if round trip do they have to be returned to the pickup point same day? To/From or Round trip Lab Sample containers are interchangeable to all areas empty containers retrieved from VAMC Baltimore or VAMC Perry Point Lab then swapped with containers at each location.
