QUOTATION FOR HAZARDOUS WASTE DISPOSAL SERVICES

Project Description: Waste disposal services for hazardous and non-hazardous materials listed in the “UH Cancer Center Kakaako Hazardous Waste Inventory” attachment.

Aloha,

The University of Hawaii Cancer Center, requests a quotation for hazardous waste disposal services.

Attached to this request is a list of chemical waste, the technical specifications required by the University for the disposal of hazardous waste, UH Terms and Conditions, special instructions and the UH Insurance Attachment. The quotation total must include all shipping and handling charges as well as any taxes that may be assessed.

At least a week prior to waste pick-up, the manifests and lab pack inventories will have to be submitted to Matthew Hiramoto at the University of Hawaii Cancer Center Environmental Health & Safety Office via email (MHiramoto@cc.hawaii.edu) for review and comment.

PLEASE SUBMIT YOUR QUOTATION NO LATER THAN 4:00 PM on Friday, February 20th, 2026. The quotation must be signed by an authorized representative of your company.

The contracted company must provide service (i.e. remove all waste listed on the inventory from University Property) by Friday, February 20th, 2026.

Vendor must submit a certificate of vendor compliance at time of quote submission.

Quotes received from vendors who do not have a certificate of vendor compliance will be disqualified.
