Pursuant to N.J.S.A. 19:44A-20.4 et seq., the City of Clifton is soliciting Fair and Open
Proposals for Professional Medical Services for a contract period of March 01,2026 to
February 28,2027. Proposals shall be endorsed “Professional Medical Services” and delivered
to Purchasing Agent Amisha Jariwala, City of Clifton, 900 Clifton Avenue, Clifton, New Jersey
07013.

REQUEST FOR PROPOSALS
PROFESSIONAL MEDICAL SERVICES
The City of Clifton is soliciting a Request for Proposal (“RFP”) to provide Pre -Employment Physicals/Medical
& Drug Testing services from professional medical service providers interested in providing the City with
necessary medical examinations and related medical services on an as-needed basis. THE TEST SITE MUST
BE WITHIN 5 MILES FROM CITY OF CLIFTON 900 CLIFTON AVE CLIFTON NJ 07013. The City
of Clifton reserves the right to award this contract in whole or in parts.
Services are being solicited from laboratory facilities which are certified under subpart C of the US Department
of Health and Human Services “Mandatory Guidelines for Federal workplace drug testing programs” (59 FR
29916,29925) for the provision of analytical toxicology services to detect and report the presence of specified
drugs in employees and candidates for employment, in compliance with laboratory analysis procedures specified
in 49 CFR Part 40,December 19,2000 and as amended thereafter, complying with applicable implementation
dates as specified in regulations. All candidates are required to comply with N.J.S.A. 10:5-31, et seq. and N.J.A.C.
17:27 as amended (Affirmative Action).
The City of Clifton will require the facility to conduct urine specimen analysis for pre-employments, random,
post-accident, reasonable suspicion and return to duty/follow up drug testing. Laboratories will be expected to
provide witnesses to defend the integrity of their analytical procedures.
I – GENERAL
QUALIFICATIONS OF LABORATORY
The successful laboratory shall have at a minimum the following qualifications: -
1. LICENSE TO DO BUSINESS IN THE STATE OF NEW JERSEY
2. CERTIFICATION BY THE U.S. DEPARTMENT OF HEALTH AND HUMAN SERVICES UNDER THE NATIONAL
LABORATORY CERTIFICATION PROGRAM (NLCP)
3. AT LEAST THREE (3) YEARS EXPERIENCE PERFORMING THE TYPES OF DRUG TESTING SERVICES SPECIFIED.
4. MUST BE ABLE TO PERFORM A BREATHALYZER ON SITE TEST AT THE TESTING CENTER.
5. ALL STRESS TESTS SHALL BE PERFORMED BY BOARD CERTIFIED CARDIOLOGIST.
II - SCOPE OF SERVICES
• The subsequent pages will address specific various departmental medical service requirements, but each
department as listed will require medical evaluations for new and existing employees on a timely basis.
Copies of the results shall be forwarded to the applicable Personnel Representative.
• Each submittal of a response is required to include an organizational chart and list the medical
professionals that will work on behalf of the responding service provider to assist the City with as-
needed requests.
• Submittals should include resumes of all medical staff (doctor’s, physician assistants, and nurses).

III – PRICING, AWARD, AND CONTRACT TERM
• Pricing shall be in the form of flat cost per candidate and shall be provided on the forms herein, five
pages in total, and each of the five pages requires signature, date, and printed name information. Each
RFP will be viewed to determine responsiveness. Responsive submissions will be evaluated by
Department Head or Designee (s). Contract will be awarded to the vendor who submits the proposal
which is determined to be the most advantageous to the City ,price and other factors considered.
• In accordance with the N.J.S.A. 40A:11-15 et seq., the term of contract shall be for one year from Date
of Award. This contract can be extended for up to two more years if successful vendor and City of
Clifton mutually agree under the same terms and conditions.
• All the Pre-employment physicals shall be conducted within three (3) working days from the date of the
request.
• The physician shall provide the City with the typed report of the examination within three (3) working
days from the date of the examination informing the City if the individual can or cannot physically
perform the required duties of his/her position.

• Submittals may be mailed, or hand delivered in a sealed envelope, with the outermost
envelope/packaging showing the Company name and the words “Professional Medical Services”, and
must be addressed to City of Clifton, Purchasing Agent, at 900 Clifton Avenue, 2nd Floor, Clifton NJ
07013.

Goods and Services Contracts (including purchase orders)
N.J.S.A. 52:32-44 imposes the following requirements on bidder/respondents and all sub-contractors that
knowingly provide goods or perform services for a bidder/respondent fulfilling this contract:
1) Business submitting the response must be registered at time of submittal, and shall provide proof by or
before award of contract
2) the bidder/respondent shall provide written notice to its sub-contractors to submit proof of business
registration to the bidder/respondent;
3) prior to receipt of final payment from a contracting agency, a bidder/respondent must submit to the
contracting agency an accurate list of all sub-contractors or attest that none was used;
4) during the term of this contract, the bidder/respondent and its affiliates shall collect and remit to the
Director, New Jersey Division of Taxation, the use tax due pursuant to the Sales and Use Tax Act,
(N.J.S.A. 54:32B-1 et seq.) on all sales of tangible personal property delivered into this State.
Construction Contracts (including public works related purchase orders)
N.J.S.A. 52:32-44 imposes the following requirements on bidder/respondents and all sub-contractors that
knowingly provide goods or perform services for a bidder/respondent fulfilling this contract:
1) the bidder/respondent shall provide written notice to its sub-contractors and suppliers to submit proof of
business registration to the bidder/respondent;
2) sub-contractors through all tiers of a project must provide written notice to their sub-contractors and
suppliers to submit proof of business registration and sub-contractors shall collect such proofs of business
registration and maintain them on file;
3) prior to receipt of final payment from a contracting agency, a bidder/respondent must submit to the
contracting agency an accurate list of all sub-contractors and suppliers or attest that none was used; and
4) during the term of this contract, the bidder/respondent and its affiliates shall collect and remit, and shall
notify all sub-contractors and their affiliates that they must collect and remit, to the Director, New Jersey
Division of Taxation, the use tax due pursuant to the Sales and Use Tax Act, (N.J.S.A. 54:32B-1 et seq.)
on all sales of tangible personal property delivered into this State.
A bidder/respondent, sub-contractor or supplier who fails to provide proof of business registration or provides
false business registration information shall be liable to a penalty of $25 for each day of violation, not to exceed
$50,000.00 for each business registration copy not properly provided or maintained under a contract with a

NOTE: Non-profit organizations are exempt – however, non-profit organizations must provide a copy
of their 501C status from the IRS or equivalent
