Produce Safety Passport Booklets

The Ohio Department of Education and Workforce ("DEW")seeks a supplier to print 30,000 4.25"x5.5" produce safety booklets with a sticker page insert. The main book will be a total of 5 front and back pages. Cover, and back pages will be 100lb glossy, internal pages will be standard copy paper and saddle sewn binding. Additionally, the booklet will need 1.5 pages that will have 6-2.5" stickers (2 stickers per page side) to be used in the book.

DEW requests a physical proof be sent before production starts. Supplier must indicated estimated production time in their quote.

All bids must include a quote document, preferably a .PDF file. The quote must include the cost of printing and shipping to DEW.
