PPO 47904 BMV 2255 5-25 ALS - OVI Refusal Arrest

BACKGROUND & OVERVIEW: The Ohio Department of Public Safety (ODPS), Bureau of Motor Vehicles (BMV), is seeking a one‑time purchase of 20,000 BMV 2255 5/25 ALS‑OVI Refusal Arrest forms. These forms are used for official documentation and must meet all quality, performance, and accessibility requirements described below. Due to critical low stock levels, immediate replenishment is necessary to ensure uninterrupted use of these legally required forms.

To accommodate the required accelerated production timeline, vendors may utilize digital printing methods, provided all quality, carbonless, and performance specifications outlined in this document are met. All forms must fully comply with ODPS standards and be delivered within thirty (30) days of purchase order issuance.

STANDARD TERMS AND CONDITIONS: The State of Ohio Standard Terms and Conditions (revised 03/31/25, attached) apply to this solicitation and will govern any resulting Contract.

EVALUATION: Bids will be evaluated in accordance with ORC 125.07. In addition, the state will multiply the bid prices by the estimated usage listed in the bid and then adding each of the totals together to arrive at a total for all items.

CONTRACT AWARD: The contract will be awarded to the lowest responsive and responsible bidder by low lot total. The low lot total will be determined by multiplying the unit prices by the estimated usage listed in the bid and then adding each of the totals together to arrive at a total for all items. Failure to bid on all items may result in the bidder being deemed not responsive.
