BACKGROUND

Family Health International (FHI 360) is an international nonprofit working to improve the health and well-being of people in the United States and around the world. PURPOSE

FHI 360 invites qualified suppliers to submit quotations for the supply and delivery of the following laboratory equipment for the operationalization of molecular testing at Vaccine Preventable Diseases - Referral Laboratory Network Sites:

1 unit/set Real-Time PCR System with gradient thermal cycler and multiplex detection capability

1 unit/set Fluorometer for nucleic acid and protein quantification

VENDOR REQUIREMENTS

The vendor must have the capability to supply the specified laboratory equipment, consumables, and supplies in the required quantities and packaging configurations.

The vendor is required to submit the following documents and information as part of their quotation:

Lot/Batch Number and Expiry Date (where applicable)

Lead Time for delivery

Certificate of Product Registration or an Authorized Distributor/Dealer Certificate

License to Operate (LTO) as a Medical Device Importer, Distributor, or Wholesaler, as applicable

Product Insert/Brochure/Data Sheet (if available)

Quality Assurance Documentation for the commodity or product being offered (e.g., Certificate of Analysis, Quality Assurance Status, or equivalent), if available

VENDOR SELECTION & CONTRACT AWARD

The final selection will be based on the best value, considering technical compliance with the specifications, product quality, vendor capability, and experience, delivery commitment, price, and payment terms.

SUBMISSION REQUIREMENTS

Complete the bid package below and submit it by email no later than August 27, 2026, at 11:59 PM, to . Bidders must submit the following documents:

Company Profile / Corporate Qualification Statement

Formal quotation on company letterhead including completed price schedule

Valid business registration documents, such as Business Permit, BIR Certificate of Registration (COR), DTI Registration, or SEC Registration, applicable

Proposed leadtime and delivery terms

Product brochure, data sheet, or technical specification sheet for each offered item

Lot/batch number and expiry date, where applicable

Certificate of Product Registration or equivalent authorized regulatory o/distributor documentation, as applicable

Valid License to Operate (LTO) as medical device importer, distributor, or wholesaler, as applicable to the items offered

Product insert, if available/applicable

Quality assurance documentation, such as Certificate of Analysis, Certificate of Conformance, or equivalent documentation, if available/applicable

RESERVED RIGHTS

All RFQ responses become the property of FHI 360, which reserves the right in its sole discretion to:

To disqualify any offer based on the offeror's failure to follow solicitation instructions.

Extend the time for submission of all RFQ responses after notification to all offerors.

Terminate or modify the RFQ process at any time and re-issue the RFQ to whomever FHI 360 deems appropriate.

FHI 360 may choose to issue multiple awards based on the solicitation.

Reserves the right to issue an award based on the initial evaluation of offers without discussion.

FHI 360 will not compensate vendors for the preparation of their response to this RFQ.

Issuing this RFQ is not a guarantee that FHI 360 will award a Master Service Agreement

FHI 360 shall not be obligated to order any minimum or maximum quantities of services or products and assumes no commitment, financial or otherwise.
