Project Description: Repair Tile Address: 1626 Vanderbilt Dr.
Contractor: Repair blocked tile drain located in Perry Lakes Estate Section 5, Perry Township/Section 27, 1626 Vanderbilt
Dr. Description of Work: The existing 12" HDPE tile drain has a confirmed blockage. Bid on the excavation, blockage
removal, and restoration of the affected tile. Scope of Work: Tile Repair: Excavate and expose the existing 12" HDPE tile
drain at the location of the blockage. Identify and remove the cause of the blockage. Repair or replace any damaged tile as
needed. Estimated repair length is 20 L.F. however contractor should include in their bid any additional footage that may be
necessary . Restore area to original grade or better. Seeding & Restoration: All disturbed ground shall be seeded with
ACSO approved seed mix and crimped with straw. Contractor is responsible for full restoration of all areas disturbed
during the course of work. Cleanup: All debris, spoil, and materials removed from the site shall be hauled away and
disposed of off site. All manner of work needed to complete the job. Estimated Repair: 20 L.F. 12" HDPE Tile
